Cardofix (Bisoprolol Fumarate) is a beta-blocker medication primarily used to treat high blood pressure (hypertension), angina (chest pain), and chronic heart failure. It works by relaxing blood vessels and slowing heart rate to improve blood flow and decrease blood pressure.
Uses & Indications
Dosage
Adults
Hypertension/Angina: Initially 5 mg once daily, may be increased to 10 mg once daily. Maximum 20 mg/day. Chronic Heart Failure: Initially 1.25 mg once daily, titrated gradually over weeks to a maximum of 10 mg once daily.
Elderly
No specific dose adjustment is usually required unless severe renal or hepatic impairment is present. Start with a lower dose.
Renal_impairment
For severe renal impairment (creatinine clearance <20 mL/min), maximum dose is 10 mg/day. Use with caution.
How to Take
Take orally once daily, preferably in the morning with or without food.
Mechanism of Action
Bisoprolol is a highly cardioselective beta-1 adrenergic receptor blocking agent. It selectively blocks beta-1 receptors in the heart, leading to decreased heart rate, reduced myocardial contractility, and lower blood pressure. It has minimal effect on beta-2 receptors in the lungs and peripheral blood vessels at therapeutic doses.

Side Effects
Contraindications
- Acute heart failure or decompensated heart failure requiring intravenous inotropic therapy
- Cardiogenic shock
- Second or third degree AV block (without a pacemaker)
- Sick sinus syndrome
- Severe bradycardia (<60 bpm prior to therapy)
- Severe hypotension
- Severe asthma or severe chronic obstructive pulmonary disease (COPD)
- Late stages of peripheral arterial occlusive disease and Raynaud's syndrome
- Untreated pheochromocytoma
- Metabolic acidosis
- Hypersensitivity to Bisoprolol or any component of the formulation
Drug Interactions
NSAIDs
May reduce the hypotensive effect of Bisoprolol.
Clonidine
Risk of rebound hypertension upon discontinuation of clonidine if Bisoprolol is not gradually withdrawn first.
Insulin and oral antidiabetics
May mask symptoms of hypoglycemia.
Antiarrhythmics (e.g., amiodarone)
Increased risk of cardiac depression and bradycardia.
Calcium channel blockers (e.g., verapamil, diltiazem)
Increased risk of bradycardia, AV block, and heart failure.

Overdose
Symptoms include bradycardia, hypotension, bronchospasm, acute cardiac insufficiency. Management involves supportive care: atropine for bradycardia, vasopressors for hypotension, bronchodilators for bronchospasm, diuretics for cardiac insufficiency.

Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Small amounts excreted in breast milk; avoid use during lactation or monitor infant for beta-blockade symptoms.
