Overview of the medicine
Metoprolol Tartrate is a beta-1 selective adrenergic receptor blocker used to treat hypertension, angina pectoris, and certain cardiac arrhythmias. It reduces heart rate and blood pressure.
Uses & Indications
Dosage
Adults
Typical initial dose for hypertension is 50-100 mg once daily or in divided doses. Max 400 mg/day.
Elderly
Similar to adults, but starting with lower doses may be considered due to potential reduced elimination and increased sensitivity.
Renal_impairment
No dose adjustment is usually required for renal impairment, but caution is advised in severe cases.
How to Take
Take orally with or immediately after food, at the same time each day. Swallow whole; do not crush or chew.
Mechanism of Action
Selectively blocks beta-1 adrenergic receptors in the heart, reducing heart rate, cardiac output, and blood pressure. It also inhibits renin release.
Pharmacokinetics
Onset
Oral: 1 hour for blood pressure reduction. IV: Minutes.
Excretion
Mainly renal, with about 95% excreted in urine as metabolites, and only a small fraction as unchanged drug.
Half life
3-7 hours.
Absorption
Rapid and complete, but significant first-pass metabolism reduces bioavailability to about 50%. Peak plasma concentrations within 1-2 hours.

Primarily hepatic via CYP2D6.

Contraindications
- Severe bradycardia
- Second- or third-degree heart block
- Cardiogenic shock
- Decompensated heart failure
- Sick sinus syndrome (unless pacemaker in place)
- Severe peripheral arterial disease
Drug Interactions
NSAIDs
May reduce antihypertensive effects.
Digoxin
Additive bradycardia effect.
Clonidine
Risk of rebound hypertension upon clonidine withdrawal.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)

Use in pregnancy only if potential benefit outweighs risk. Excreted in breast milk, use with caution or consider alternatives; monitor infant for beta-blockade effects.
