Description
Overview of the medicine
Carlina 5 mg tablet is an antihypertensive medication primarily used to manage high blood pressure. It belongs to the class of beta-adrenergic blockers, which work by relaxing blood vessels and slowing heart rate to improve blood flow.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, may be increased to 10 mg once daily after 1-2 weeks if necessary. Maximum dose is 20 mg once daily.
Elderly
No specific dose adjustment required, but start with the lowest effective dose (5 mg once daily) and monitor closely.
Renal_impairment
For creatinine clearance <30 mL/min, maximum dose should not exceed 10 mg once daily.
How to Take
Carlina 5 mg tablets should be swallowed whole with water, preferably in the morning before breakfast. Do not chew or crush the tablet.
Mechanism of Action
Carlina selectively blocks beta-1 adrenergic receptors, primarily located in the heart. This blockade reduces heart rate, myocardial contractility, and cardiac output, leading to a decrease in blood pressure. It also inhibits the release of renin from the kidneys, further contributing to its antihypertensive effects.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Mainly renal, with some excretion in feces
Half life
Approximately 8-10 hours
Absorption
Rapidly and completely absorbed from the gastrointestinal tract; bioavailability is approximately 80-90%.
Metabolism
Primarily hepatic, extensive first-pass metabolism
Side Effects
Contraindications
- Severe bradycardia (<50 beats/min)
- Cardiogenic shock
- Uncontrolled heart failure
- Second or third-degree atrioventricular block
- Sick sinus syndrome
Drug Interactions
Digoxin
Increased risk of bradycardia.
NSAIDs (e.g., Ibuprofen, Indomethacin)
May reduce the antihypertensive effect of Carlina.
Antidiabetic drugs (e.g., Insulin, Sulfonylureas)
Carlina may mask symptoms of hypoglycemia and potentiate hypoglycemic effects.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and hypotension. Avoid concomitant use.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe bradycardia, hypotension, acute heart failure, and bronchospasm. Management involves symptomatic and supportive measures. Atropine, vasopressors, glucagon, and cardiac pacing may be necessary.
Pregnancy & Lactation
Pregnancy Category C. Carlina should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, caution should be exercised when Carlina is administered to a nursing woman.
