Description
Overview of the medicine
Carvista 625 mg Tablet is a combination antibiotic containing Amoxicillin and Clavulanic Acid. It is used to treat a wide range of bacterial infections including those of the respiratory tract, urinary tract, skin and soft tissue, and ear, nose, and throat. Amoxicillin is a penicillin-type antibiotic that fights bacteria, while Clavulanic Acid helps Amoxicillin work more effectively by preventing bacterial resistance.
Uses & Indications
Dosage
Adults
The usual adult dose for Carvista 625 mg (Amoxicillin 500 mg + Clavulanic Acid 125 mg) is one tablet taken every 8 hours (three times daily) or as directed by the physician. Dosage may vary based on the severity and type of infection.
Elderly
No specific dose adjustment is generally required in elderly patients with normal renal function. Monitor renal function.
Renal_impairment
Dose adjustment is necessary for patients with renal impairment. For creatinine clearance between 10-30 mL/min, 625 mg every 12 hours; for creatinine clearance <10 mL/min, 625 mg every 24 hours. Hemodialysis patients should receive an additional dose after dialysis.
How to Take
Take the tablet whole with a glass of water. It can be taken with or without food, but taking it at the start of a meal may reduce potential gastrointestinal discomfort. Do not chew or crush the tablet. Complete the full course of treatment as prescribed by your doctor, even if you feel better.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. Clavulanic Acid is a beta-lactamase inhibitor that inactivates beta-lactamase enzymes produced by some bacteria, which would otherwise degrade Amoxicillin. This combination broadens the spectrum of Amoxicillin's activity against beta-lactamase-producing bacteria.

Onset
Within 1-2 hours of oral administration.
Excretion
Mainly excreted by the kidneys (glomerular filtration and tubular secretion). Approximately 50-70% of Amoxicillin and 25-40% of Clavulanic Acid are excreted unchanged in urine within 6-8 hours.
Half life
Approximately 1-1.5 hours for both Amoxicillin and Clavulanic Acid.
Absorption
Both Amoxicillin and Clavulanic Acid are well absorbed from the gastrointestinal tract after oral administration. Absorption is not significantly affected by food. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Amoxicillin is primarily excreted unchanged. Clavulanic Acid is extensively metabolized to inactive metabolites.
Side Effects
Contraindications
- History of severe hypersensitivity reaction to beta-lactam antibiotics (e.g., anaphylaxis to penicillins or cephalosporins)
- History of amoxicillin/clavulanic acid-associated jaundice or hepatic dysfunction
- Patients with infectious mononucleosis (due to risk of rash)
Drug Interactions
Probenecid
Decreases renal tubular secretion of Amoxicillin, resulting in increased and prolonged blood levels of Amoxicillin. Concomitant use is not recommended.
Allopurinol
Increased incidence of rash, particularly in patients with hyperuricemia.
Methotrexate
May reduce the excretion of methotrexate, potentially increasing its toxicity.
Warfarin and other oral anticoagulants
Increased prothrombin time (INR) in patients receiving Amoxicillin/Clavulanic Acid and oral anticoagulants. Careful monitoring is necessary.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastrointestinal symptoms (e.g., abdominal pain, vomiting, diarrhea) and fluid and electrolyte imbalance may be prominent. Convulsions may occur in patients with severely impaired renal function or in those receiving high doses. Treatment should be symptomatic and supportive, with attention to fluid and electrolyte balance. Amoxicillin/Clavulanic Acid can be removed from circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. While studies in animals have not shown harm to the fetus, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Both Amoxicillin and Clavulanic Acid are excreted in human milk. Caution should be exercised when administered to a nursing mother.
