Description
Overview of the medicine
Vinpocetine is a synthetic derivative of the Vinca minor alkaloid vincamine. It is used to improve cerebral blood flow and metabolism, and has neuroprotective effects. Cavinton 5 mg tablet is primarily indicated for various neurological conditions related to cerebrovascular insufficiency.
Uses & Indications
Dosage
Adults
Usually 5-10 mg three times daily. The dose can be adjusted based on clinical response and tolerability.
Elderly
No specific dose adjustment is generally required for elderly patients, same as adults.
Renal_impairment
No specific dose adjustment is generally required for mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Take tablets orally, preferably after meals, to enhance absorption.
Mechanism of Action
Vinpocetine selectively increases cerebral blood flow and metabolism by inhibiting phosphodiesterase type 1 (PDE1), leading to increased cGMP levels in vascular smooth muscle. It also improves red blood cell deformability, reduces blood viscosity, and has antioxidant and neuroprotective properties. It enhances glucose and oxygen utilization in the brain.
Pharmacokinetics
Onset
Clinical effects may take days to weeks to become apparent, depending on the condition being treated.
Excretion
Excreted renally (approximately 60%) and fecally (approximately 40%) as metabolites and unchanged drug.
Half life
Approximately 4.5-5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 1 hour. Bioavailability is low (approximately 7%) due to extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by esterases, to form active and inactive metabolites. The main metabolite is apovincaminic acid (AVA).
Side Effects
Contraindications
- Hypersensitivity to Vinpocetine or any components of the formulation.
- Acute cerebral hemorrhage.
- Severe ischemic heart disease or severe arrhythmia.
