Description
Overview of the medicine
Vinpocetine is a synthetic derivative of the Vinca minor alkaloid vincamine, used to treat various cerebrovascular disorders and cognitive impairments. It works by improving cerebral blood flow and neuronal metabolism.
Uses & Indications
Dosage
Adults
Typically 10-20 mg diluted in 500-1000 ml infusion solution (isotonic saline or glucose), administered intravenously over 60-90 minutes, 1-2 times daily. Treatment usually lasts for 10-14 days, followed by oral maintenance therapy.
Elderly
No specific dosage adjustment usually required, but careful monitoring is advisable.
Renal_impairment
No specific dosage adjustment recommended.
How to Take
Administer as an intravenous infusion only, diluted in isotonic saline or glucose solution. Do not administer undiluted or as an intramuscular injection. The infusion rate should not exceed 80 drops/minute.
Mechanism of Action
Vinpocetine acts as a selective cerebral vasodilator, improves cerebral microcirculation, and enhances glucose and oxygen utilization in the brain. It inhibits phosphodiesterase 1 (PDE1), leading to increased cGMP levels, which results in relaxation of cerebral smooth muscles and improved neuronal metabolism. It also has neuroprotective effects.
Pharmacokinetics
Onset
Rapid (within minutes for IV infusion).
Excretion
Mainly renal and fecal.
Half life
Approximately 2-3 hours.
Absorption
Rapidly available after intravenous administration.
Metabolism
Primarily hepatic, metabolized to apovincaminic acid (AVA).
Side Effects
Contraindications
- Known hypersensitivity to vinpocetine or other vinca alkaloids.
- Acute cerebral hemorrhage.
- Severe coronary artery disease.
- Severe arrhythmias.
- Pregnancy and lactation.
Drug Interactions
Antihypertensives
May potentiate hypotensive effects, leading to a drop in blood pressure.
QT-prolonging drugs
Use with caution due to potential additive effects on QT interval.
Anticoagulants/Antiplatelets
May increase the risk of bleeding.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms of overdose may include hypotension and cardiac arrhythmias. Management is symptomatic and supportive, with close monitoring of vital signs and cardiac function.
Pregnancy & Lactation
Contraindicated during pregnancy due to lack of sufficient safety data and potential for fetal harm. Also contraindicated during lactation as vinpocetine is excreted into breast milk.
