Cazar 20 mg Capsule contains Omeprazole, a proton pump inhibitor used to treat conditions caused by excess stomach acid, such as heartburn, acid reflux, ulcers, and Zollinger-Ellison syndrome. It works by decreasing the amount of acid produced in the stomach.
Uses & Indications
Dosage
Adults
Typical dosage for GERD and ulcers is 20 mg once daily for 4-8 weeks. For severe cases or Zollinger-Ellison syndrome, doses may be higher (e.g., 40-60 mg daily or more).
Elderly
No specific dosage adjustment is typically required, but caution is advised in elderly patients with hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Usually taken once daily in the morning, preferably before a meal. Swallow the capsule whole with water; do not chew or crush.
Mechanism of Action
Omeprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells. This inhibition prevents the final step of acid secretion, thereby reducing gastric acid production.
Pharmacokinetics
Onset
Acid secretion inhibition occurs within 1 hour, maximum effect within 2 hours. Full acid suppression may take several days of therapy.
Excretion
Approximately 80% of metabolites are excreted in the urine and approximately 20% in the feces (bile secretion).
Half life
Plasma elimination half-life is approximately 0.5 to 1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly but variably absorbed from the gastrointestinal tract, primarily in the small intestine. Bioavailability is approximately 30-40% but increases with repeated dosing due to reduced first-pass metabolism.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to Omeprazole or any component of the formulation
- Concomitant use with Nelfinavir
Drug Interactions
Warfarin
Increased INR/prothrombin time; monitor anticoagulant effects closely.
Clopidogrel
May reduce the antiplatelet effect of Clopidogrel by inhibiting CYP2C19. Consider alternative PPIs or antiplatelet therapy.
Methotrexate
May increase serum levels of methotrexate, particularly high-dose methotrexate; consider temporary PPI withdrawal.
