Ceftriaxone is a broad-spectrum third-generation cephalosporin antibiotic used to treat various bacterial infections, including severe and life-threatening ones. It is administered via injection.
Uses & Indications
Dosage
Adults
Usual adult dose is 1-2g once daily, or 0.5-1g every 12 hours, depending on the severity of the infection. Maximum dose is 4g daily.
Elderly
No specific dose adjustment is required for the elderly unless there is severe renal or hepatic impairment.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. For severe impairment (creatinine clearance <10 mL/min), dosage should not exceed 2g daily. Dialysis patients may not require supplemental doses.
How to Take
After reconstitution, Ceftriaxone injection can be administered either intravenously (IV) as a slow infusion over 30 minutes or intramuscularly (IM) by deep injection into a large muscle mass. Do not inject more than 1g into a single IM site.
Mechanism of Action
Ceftriaxone works by inhibiting the synthesis of the bacterial cell wall, leading to bacterial lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall.

Side Effects
Contraindications
- Known hypersensitivity to Ceftriaxone, any other cephalosporin, or to any of the excipients.
- In neonates (≤28 days) with hyperbilirubinemia, especially premature neonates, due to the risk of bilirubin encephalopathy.
- Concomitant use with calcium-containing solutions in neonates (≤28 days).
Drug Interactions
Aminoglycosides
May increase the risk of nephrotoxicity and ototoxicity.
Calcium-containing solutions
Concomitant use is contraindicated in neonates due to precipitation risk. In other age groups, a sequential administration with flushing is recommended.
Loop Diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, increasing the risk of bleeding.
Storage
Store powder for injection below 30°C, protected from light and moisture. Reconstituted solutions should be used immediately or stored as per manufacturer's guidelines (e.g., for 24 hours at 2-8°C).
