Overview of the medicine
Cefixime is a broad-spectrum, third-generation cephalosporin antibiotic used to treat various bacterial infections. It is effective against a wide range of Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg orally once daily, or 200 mg every 12 hours. Duration depends on the infection and may range from 3 to 14 days.
Elderly
No specific dose adjustment is required in elderly patients with normal renal function; however, renal function should be monitored, and dose adjusted if impairment is present.
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 300 mg daily. For creatinine clearance < 20 mL/min, the dose should be 200 mg daily.
How to Take
Cebuten 400 mg capsule should be taken orally. It can be taken with or without food. To maximize absorption and minimize gastrointestinal upset, some patients prefer to take it with food. Complete the full prescribed course of treatment.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall.
Pharmacokinetics
Onset
Clinical effect typically observed within 24-48 hours of initiation.
Excretion
Approximately 50% of an absorbed dose is excreted unchanged in the urine over 24 hours. Some biliary excretion also occurs. Renal impairment prolongs half-life.
Half life
The plasma half-life is typically 3-4 hours, allowing for once-daily or twice-daily dosing.
Absorption
Oral absorption is about 40-50%, not significantly affected by food. Peak plasma concentrations are achieved 2-6 hours after administration.
Metabolism
Cefixime is not extensively metabolized; about 10% may be metabolized. No active metabolites are formed.
Side Effects
Contraindications
- •Hypersensitivity to cefixime, other cephalosporins, or any component of the formulation.
- •History of severe hypersensitivity reaction to penicillin (e.g., anaphylaxis), due to potential for cross-reactivity.
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by inhibiting its renal excretion.
Carbamazepine
Concomitant administration has been shown to result in increased carbamazepine levels.
Warfarin and other anticoagulants
Increased prothrombin time (INR) with concomitant use; monitoring of coagulation parameters is recommended to adjust anticoagulant dose if necessary.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Supportive treatment of symptoms is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in human milk in small concentrations; caution should be exercised when administered to a nursing woman.
