Description
Overview of the medicine
Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. It works by reducing the production of prostaglandins.
Uses & Indications
Dosage
Adults
100 mg twice daily, preferably with food.
Elderly
The lowest effective dose should be used. Monitoring of renal and hepatic function is advised.
Renal_impairment
No dose adjustment is usually required in mild renal impairment. In moderate to severe renal impairment, dose reduction or avoidance may be necessary. Use with caution.
Hepatic_impairment
A reduced dose of 100 mg once daily is recommended in mild to moderate hepatic impairment. Not recommended in severe hepatic impairment.
How to Take
Take orally, preferably with or after food to minimize gastrointestinal upset. Swallow the tablet whole with a sufficient amount of liquid.
Mechanism of Action
Aceclofenac primarily acts by inhibiting the enzyme cyclooxygenase (COX), which is involved in the synthesis of prostaglandins, inflammatory mediators. It demonstrates higher selectivity for COX-2 than COX-1, which may contribute to a better gastrointestinal tolerability profile compared to some other NSAIDs.

Onset
Within 30-60 minutes.
Excretion
Approximately two-thirds of the administered dose is excreted in the urine, mainly as hydroxylated and conjugated metabolites, and one-third in the faeces.
Half life
Approximately 4-4.3 hours.
Absorption
Well absorbed orally, peak plasma concentrations reached within 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2C9, to 4'-hydroxyaceclofenac and other metabolites. Also undergoes conjugation.
Side Effects
Contraindications
- •Hypersensitivity to aceclofenac or any excipients
- •Patients with a history of asthma, urticaria, or acute rhinitis precipitated by aspirin or other NSAIDs
- •Active or recurrent peptic ulcer/haemorrhage (two or more distinct episodes of proven ulceration or bleeding)
- •Severe heart failure, severe hepatic insufficiency, or severe renal insufficiency
- •Third trimester of pregnancy
- •History of gastrointestinal bleeding or perforation related to previous NSAID therapy
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased plasma methotrexate levels.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Other NSAIDs and corticosteroids
Increased risk of gastrointestinal adverse effects.
Selective Serotonin Reuptake Inhibitors (SSRIs)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhoea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus and fainting. Acute renal failure and liver damage are possible. Management involves symptomatic and supportive treatment. Gastric lavage or activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C (first two trimesters), D (third trimester). Should be avoided during pregnancy, especially in the third trimester due to potential adverse effects on the fetus (e.g., premature closure of ductus arteriosus). Use with caution during lactation; consult a doctor.
