Overview of the medicine
Cefivic-DS 200 mg Suspension is an oral antibiotic belonging to the third-generation cephalosporin class. It is used to treat a wide range of bacterial infections in children, including respiratory tract infections, urinary tract infections, and ear infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Not typically used in this suspension form for adults. For children 6 months to 12 years: 8 mg/kg/day once daily or in two divided doses (4 mg/kg every 12 hours) for 7-14 days depending on infection severity. Maximum 400 mg/day.
Elderly
Not applicable for this pediatric formulation. For adult formulations, dosage adjustment may be necessary in elderly patients with significant renal impairment.
Renal_impairment
For children with creatinine clearance <60 mL/min, dosage adjustment is required. Consult physician for specific recommendations.
How to Take
Shake the bottle well before each use. Administer orally with a measuring spoon or oral syringe. Can be taken with or without food. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Cefixime, a bactericidal antibiotic, works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. Cell lysis is then mediated by bacterial cell wall autolytic enzymes such as autolysins; it is possible that cefixime interferes with an autolysin inhibitor.
Pharmacokinetics
Onset
Peak plasma concentration reached within 2-6 hours after oral administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Some excretion occurs via bile/feces.
Half life
Average plasma half-life is 3-4 hours, but can extend up to 9 hours in some cases.
Absorption
Approximately 30-50% orally absorbed. Absorption may be delayed but not significantly decreased by food.
Metabolism
Not extensively metabolized; primarily eliminated as unchanged drug.
Side Effects
Contraindications
- •Hypersensitivity to cefixime or any other cephalosporin antibiotics

Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. Monitor INR/PT closely.
Probenecid
Increases cefixime plasma levels by decreasing renal tubular secretion.
Carbamazepine
May increase carbamazepine levels.
Oral contraceptives
May reduce the efficacy of oral contraceptives (though clinically significant interaction is not well established for all cephalosporins, caution is advised).
Storage
Store dry powder below 30°C, protected from light and moisture. Store reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Excreted in small amounts in breast milk; caution should be exercised when administered to a nursing woman.
