Description
Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat a variety of bacterial infections, including those of the ear, urinary tract, and respiratory tract. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing over 50 kg: 200 mg once daily or 100 mg twice daily. Severe infections: 400 mg once daily or 200 mg twice daily. For suspension, dosage is typically weight-based for children.
Elderly
No dosage adjustment necessary unless renal function is severely impaired.
Renal_impairment
For creatinine clearance < 60 mL/min, dosage adjustment required. For creatinine clearance < 20 mL/min, 200 mg once daily. For suspension, specific ml/kg guidance might be needed.
How to Take
Administer orally. Shake the suspension well before each use. Can be taken with or without food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of peptidoglycan synthesis, which ultimately causes bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours after administration.
Excretion
Primarily renal excretion (approximately 50% unchanged drug within 24 hours). Also, biliary excretion.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can delay absorption but does not significantly alter total absorption.
Metabolism
Not extensively metabolized; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to cefixime or other cephalosporin antibiotics
- History of serious hypersensitivity reactions to penicillin (due to potential cross-reactivity)
Drug Interactions
Nifedipine
May increase cefixime levels.
Probenecid
Increases cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
Elevated carbamazepine levels reported.
Oral anticoagulants (e.g., Warfarin)
Increased prothrombin time (PT) with or without bleeding. Monitor INR.
Storage
Store dry powder below 30°C, protected from light and moisture. Reconstituted suspension should be stored at 2-8°C (refrigerated) and not frozen. Discard after specified period.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution during lactation.

- INR/PT in patients on oral anticoagulants
- Complete blood count (CBC) during prolonged therapy
Doctor Notes
- Consider renal function in elderly and pediatric patients for appropriate dosing.
- Educate patients on the importance of completing the full course.
- Monitor for signs of allergic reactions, especially in patients with penicillin allergy.
Patient Guidelines
- Complete the full course of treatment, even if symptoms improve.
- Do not share this medication with others.
- Shake the suspension well before each use.
- Store at room temperature away from moisture and heat.
Missed Dose Advice
