Description
Overview of the medicine
Ceforan 100 mg tablet contains Cefixime, a third-generation cephalosporin antibiotic used to treat various bacterial infections including those of the respiratory tract, urinary tract, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
Adults and children over 12 years or weighing more than 50 kg: 200-400 mg daily, given as a single dose or in two divided doses. For uncomplicated gonorrhea, a single 400 mg dose.
Elderly
No dosage adjustment needed unless severe renal impairment is present.
Renal_impairment
Creatinine clearance 21-60 mL/min: 75% of standard dose. Creatinine clearance <20 mL/min: 50% of standard dose.
How to Take
Ceforan 100 mg tablet can be taken orally with or without food. To minimize gastric upset, it can be taken with food. The tablet should be swallowed whole with water.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the peptidoglycan synthesis that is crucial for maintaining the bacterial cell wall integrity. This leads to cell lysis and bacterial death.
Pharmacokinetics
Onset
Clinical improvement often seen within 24-48 hours.
Excretion
Primarily excreted unchanged in urine (about 50%) and partially through bile.
Half life
3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food slightly delays absorption but does not significantly affect the extent of absorption.
Metabolism
Minimal metabolism in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or other cephalosporin antibiotics
- Known hypersensitivity to penicillin (due to potential cross-reactivity)
Drug Interactions
Warfarin
May increase prothrombin time (PT) and International Normalized Ratio (INR), increasing bleeding risk. Close monitoring of PT/INR is recommended.
Probenecid
Increases cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
Increased carbamazepine levels have been reported with concomitant cefixime administration.
Oral Contraceptives
May reduce efficacy of oral contraceptives (use additional birth control methods).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy: Category B. Studies in animals have shown no evidence of harm to the fetus. Use only if clearly needed. Lactation: Excreted in very small amounts in breast milk. Use with caution during breastfeeding.
