Cefotil Plus 125 mg Suspension is an antibiotic used to treat a wide range of bacterial infections. It belongs to the cephalosporin class of antibiotics and works by stopping the growth of bacteria. Dosage
Adults
This 125 mg suspension is typically not used for adults. Adult dosage usually requires higher strengths (e.g., 250 mg or 500 mg tablets). For children 3 months to 12 years: For most infections, 125 mg twice daily or 10 mg/kg twice daily (max 250 mg twice daily). For otitis media or more severe infections: 250 mg twice daily or 15 mg/kg twice daily (max 500 mg twice daily).
Elderly
Dosage adjustment not generally required in elderly patients unless there is significant renal impairment.
Renal_impairment
Dosage should be adjusted based on creatinine clearance (e.g., for CrCl 10-29 mL/min, administer standard dose every 24 hours; for CrCl <10 mL/min, administer standard dose every 48 hours).
How to Take
Administer orally. Shake the suspension well before each use. It is recommended to take Cefuroxime Axetil with food to enhance absorption and minimize gastrointestinal upset.
Mechanism of Action
Cefuroxime Axetil is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This binding prevents the cross-linking of peptidoglycan chains, leading to the disruption of the bacterial cell wall and eventual cell lysis.

Peak plasma concentrations are achieved approximately 2-3 hours after oral administration.
Excretion
Primarily excreted unchanged by the kidneys, via glomerular filtration and tubular secretion.
Half life
Approximately 1.2-1.6 hours.
Absorption
Well absorbed from the gastrointestinal tract, especially when taken with food. Bioavailability is increased in the presence of food.
Metabolism
Rapidly hydrolyzed by esterases in the intestinal mucosa and blood to release active cefuroxime. It is not significantly metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, any other cephalosporin antibiotic, or any component of the formulation.
- History of severe hypersensitivity (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agents (e.g., penicillins, carbapenems, monobactams).
Drug Interactions
Probenecid
Concomitant administration with probenecid increases the area under the serum concentration versus time curve by 50%.
Oral Contraceptives
Cefuroxime Axetil may affect gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives.
Antacids/H2 blockers/PPIs
Drugs that reduce gastric acidity may reduce the bioavailability of cefuroxime axetil and should be used with caution.
Storage
Store dry powder below 30°C, protected from light and moisture. Store reconstituted suspension in a refrigerator (2-8°C) and discard after 10 days. Do not freeze.
Overdose
Overdose of cephalosporins can lead to cerebral irritation, convulsions, or other neurological sequelae. Treatment is supportive. Hemodialysis and peritoneal dialysis can remove cefuroxime from the body.

Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Cefuroxime is excreted in breast milk in small amounts. Use during pregnancy and lactation should be done with caution and only if clearly needed after consulting a doctor.
