Description
Overview of the medicine
Cefotime is a second-generation cephalosporin antibiotic used to treat various bacterial infections, primarily through injection.
Uses & Indications
Dosage
Adults
1-2 g intravenously (IV) or intramuscularly (IM) every 8-12 hours. Max 6 g/day.
Elderly
Dosage adjustment may be required based on renal function.
Renal_impairment
Dosage adjustment required; e.g., for CrCl < 20 mL/min, 1 g every 24 hours.
How to Take
For IV administration, reconstitute with sterile water for injection and infuse slowly over 20-30 minutes. For IM, reconstitute and inject deep into a large muscle mass.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes after IV administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours.
Absorption
Administered intravenously or intramuscularly, resulting in complete absorption.
Metabolism
Minimal metabolism in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefotime or other cephalosporins
- History of severe hypersensitivity reactions to any beta-lactam antibiotics
Drug Interactions
Probenecid
May increase cefotime plasma concentrations by decreasing renal tubular secretion.
Loop diuretics
May increase cephalosporin levels.
Aminoglycosides
Increased risk of nephrotoxicity.
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose may cause convulsions, especially in patients with renal impairment. Treatment is symptomatic and supportive; hemodialysis may be helpful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Cefotime is excreted in breast milk in small amounts; use with caution during lactation.
