Description
Overview of the medicine
Ceftibuten is a broad-spectrum, semi-synthetic, third-generation cephalosporin antibiotic. It's used to treat various bacterial infections, including those of the respiratory tract, ear, and urinary tract. It works by inhibiting bacterial cell wall synthesis.
Uses & Indications
Dosage
Adults
Adults: 400 mg once daily for 5-10 days depending on the infection. (Suspension generally for pediatric use, adults typically use capsules)
Elderly
No specific dosage adjustment usually needed unless renal impairment is present.
Renal_impairment
Dosage adjustment required based on creatinine clearance (CrCl). For CrCl < 30 mL/min, 200 mg once daily or 400 mg every other day.
How to Take
Oral administration. Shake well before use. Can be taken with or without food, though taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Varies depending on the type and severity of infection.
Excretion
Primarily via kidneys (renal excretion), with about 60% recovered in urine within 24 hours.
Half life
Approximately 2-4 hours, with slightly longer half-life in pediatric patients.
Absorption
Well absorbed orally. Food may delay peak plasma concentrations (Tmax) but does not significantly affect the extent of absorption (AUC).
Metabolism
Minimally metabolized; primarily excreted as unchanged drug.
Side Effects
Contraindications
- •Hypersensitivity to ceftibuten or other cephalosporin antibiotics.
- •History of severe hypersensitivity reactions (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Antacids
May decrease the absorption of ceftibuten.
Probenecid
May increase ceftibuten plasma concentrations and prolong its half-life by decreasing renal tubular secretion.
Oral Anticoagulants (e.g., Warfarin)
Concomitant use may increase the anticoagulant effect, requiring closer monitoring of INR.
Storage
Store dry powder at room temperature (below 30°C), protected from light and moisture. After reconstitution, store in a refrigerator (2-8°C) and discard any unused portion after 14 days. Do not freeze.
Overdose
In cases of overdose, symptomatic and supportive treatment is recommended. Hemodialysis may be useful in reducing serum ceftibuten concentrations.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use only if clearly needed. Ceftibuten is excreted in breast milk in small amounts; use with caution in nursing mothers.
