Overview of the medicine
Ceftrix 1 gm Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic used to treat various bacterial infections, including severe and hospital-acquired infections. It is administered intravenously or intramuscularly.
Uses & Indications
Dosage
Adults
1-2 gm once daily (up to 4 gm/day in severe cases), IV or IM.
Elderly
No dosage adjustment necessary unless severe renal/hepatic impairment.
Renal_impairment
Generally no dosage adjustment needed for mild to moderate impairment. For severe impairment (CrCl <10 mL/min), max 2 gm/day. Monitor plasma concentration.
How to Take
Ceftriaxone injection can be administered intravenously (IV) slowly over 30 minutes or intramuscularly (IM) deep into a large muscle. For IM, not more than 1 gm per injection site. Reconstitute with appropriate diluent (sterile water for injection, lidocaine for IM).
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bactericidal action. It is active against a wide range of Gram-positive and Gram-negative bacteria.

Onset
Within 30 minutes (IV), 2-3 hours (IM) for peak plasma concentration.
Excretion
Approximately 50-60% excreted unchanged in urine, 40-50% excreted unchanged in bile/feces.
Half life
Approximately 5.8 to 8.7 hours (prolonged in neonates and elderly).
Absorption
Rapid and complete absorption after IM administration. Bioavailability is nearly 100%.
Metabolism
Limited hepatic metabolism; primarily forms inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to ceftriaxone or any other cephalosporin
- History of severe hypersensitivity reaction to any beta-lactam antibiotic (e.g., penicillin)
- Neonates with hyperbilirubinemia, especially premature neonates (due to displacement of bilirubin from albumin binding)
Drug Interactions
Aminoglycosides
Synergistic effect against some organisms. Administer separately.
Calcium-containing solutions
Concomitant use with ceftriaxone is contraindicated in neonates due to risk of precipitation. In other age groups, administer sequentially with adequate flush.
Loop diuretics (e.g., Furosemide)
May increase nephrotoxicity, especially in patients with pre-existing renal impairment.
Oral anticoagulants (e.g., Warfarin)
May increase anticoagulant effect and risk of bleeding. Monitor INR.
Storage
Store below 30°C. Protect from light. Keep reconstituted solution at room temperature for up to 6 hours or in a refrigerator (2-8°C) for up to 24 hours. Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and convulsions. Management is primarily supportive and symptomatic. Hemodialysis is not effective for removal.

Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in low concentrations in breast milk; caution advised when administering to a nursing woman.
