Overview of the medicine
Cellcept (Mycophenolate Mofetil) is an immunosuppressant medication used to prevent organ rejection in patients who have received a kidney, heart, or liver transplant. It works by suppressing the body's immune system, which helps prevent it from attacking the transplanted organ.
Uses & Indications
Dosage
Adults
Renal Transplant: 1 g orally/IV twice daily. Cardiac Transplant: 1.5 g orally/IV twice daily. Hepatic Transplant: 1.0-1.5 g orally/IV twice daily.
Elderly
No specific dosage adjustment required, but monitor for adverse effects due to potential for reduced renal/hepatic function.
Renal_impairment
For severe chronic renal impairment (GFR < 25 mL/min), doses should not exceed 1 g twice daily, and patients should be carefully monitored.
How to Take
Cellcept can be taken with or without food, but consistency is important. Take at the same time each day. Do not crush, chew, or open capsules/tablets. Administer IV infusion slowly over no less than 2 hours.
Mechanism of Action
Mycophenolate mofetil is rapidly converted to mycophenolic acid (MPA), which is a potent, selective, uncompetitive, and reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH). This inhibition blocks the de novo pathway of guanosine nucleotide synthesis, to which lymphocytes are critically dependent for proliferation, thereby exhibiting cytostatic effects on lymphocytes.
Pharmacokinetics
Onset
Immunosuppression effects develop over days to weeks, not acutely measured.
Excretion
MPAG is primarily excreted in the urine and bile. A small amount of MPA is also excreted unchanged in the urine.
Half life
Plasma half-life of MPA is approximately 16-18 hours.
Absorption
Rapid and extensive absorption after oral administration, converted to MPA. Oral bioavailability of MPA is about 94%.
Metabolism
MPA is primarily metabolized by glucuronosyltransferase (UGT) to form the inactive phenolic glucuronide of MPA (MPAG).

Contraindications
- •Hypersensitivity to mycophenolate mofetil, mycophenolic acid, or any component of the formulation.
- •Pregnancy (unless no suitable alternative due to significant risk of birth defects).
- •Lactation.

Antacids/Cholestyramine: May reduce MPA absorption, decreasing efficacy.

Acyclovir/Ganciclovir: Increased plasma concentrations of both drugs due to competition for tubular secretion, increasing toxicity.

Live vaccines: Avoid due to immunosuppression and risk of vaccine-induced infection.

Azathioprine: Concomitant use is not recommended due to potential for severe bone marrow suppression.
Storage
Store at room temperature (20-25°C or 68-77°F). Protect from moisture and light. Keep bottles tightly closed. Oral suspension should be refrigerated after reconstitution and discarded after recommended period. Keep out of reach of children.
Overdose
Overdose can lead to severe myelosuppression (leukopenia, neutropenia), gastrointestinal disturbances, and increased risk of infection. Management is supportive; hemodialysis can remove the inactive metabolite MPAG but not MPA effectively. Cholestyramine may reduce MPA levels by interrupting enterohepatic recirculation.
Pregnancy & Lactation
Pregnancy Category D. Contraindicated due to high risk of birth defects (e.g., ear, heart, facial anomalies) and miscarriage. Use highly effective contraception during treatment and for 6 weeks after stopping. Avoid during lactation as MPA is excreted in breast milk and may harm the infant.
