Uses & Indications
Dosage
Adults
Not typically used for adults in this strength; adult doses are generally higher (e.g., 500mg or 1g tablets).
Elderly
No specific adjustment needed for elderly unless hepatic or renal impairment is present.
Renal_impairment
Use with caution; dose reduction may be necessary in severe renal impairment (e.g., CrCl < 10 ml/min).
Hepatic_impairment
Use with caution; dose reduction or avoidance may be necessary in severe hepatic impairment.
Pediatric_1_5_years
120-250 mg (5-10 ml) 2-4 times daily
Pediatric_2_3_months
60 mg (2.5 ml) 2-4 times daily (only on doctor's advice)

How to Take
Orally. Shake the bottle well before use. Administer with a measuring spoon or oral syringe.
Mechanism of Action
Paracetamol is thought to act primarily by inhibiting prostaglandin synthesis in the central nervous system (CNS), leading to analgesic and antipyretic effects.
Pharmacokinetics
Onset
30-60 minutes
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates, with less than 5% excreted as unchanged drug.
Half life
1-4 hours
Absorption
Readily absorbed from the gastrointestinal tract, peak plasma concentrations typically occur within 30-60 minutes.
Metabolism
Primarily hepatic metabolism via conjugation with glucuronic acid and sulfate. A small amount is metabolized by cytochrome P450 to N-acetyl-p-benzoquinone imine (NAPQI), which is then detoxified by glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any component of the formulation
- Severe hepatic impairment
Drug Interactions
Alcohol
Increased risk of hepatotoxicity, especially with chronic excessive use.
Warfarin
Increased anticoagulant effect with prolonged high doses of paracetamol.
Carbamazepine, Phenytoin
Increased risk of hepatotoxicity due to enzyme induction.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Hepatotoxicity is the most serious consequence of paracetamol overdose. Symptoms include nausea, vomiting, abdominal pain, and liver damage. Treatment involves gastric lavage, activated charcoal, and N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Generally considered safe for use in pregnancy and lactation when used at recommended doses, but always consult a doctor.
