Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections. It is effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
Not typically used for adults at this suspension strength; adults usually take higher dose tablets. Consult physician.
Renal_impairment
Dosage adjustment is required for patients with creatinine clearance < 60 mL/min. Consult physician for specific guidelines.
Children (6 months to 10 years)
8 mg/kg once daily or 4 mg/kg every 12 hours. Typically, for a 250 mg/5 mL suspension: 5 mL (250 mg) once daily or 2.5 mL (125 mg) every 12 hours, depending on weight, age, and severity of infection, or as directed by physician. Treatment duration is usually 7-14 days.
How to Take
Administer orally with or without food. Shake the bottle well before each use. Use the provided measuring spoon or cup for accurate dosing.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Approximately 50-55% of the absorbed dose is excreted unchanged in the urine over 24 hours. Small amounts are excreted in bile and feces.
Half life
Plasma half-life is typically 3-4 hours.
Absorption
Approximately 40-50% absorbed orally. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Minimal hepatic metabolism, primarily excreted as unchanged drug.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reaction to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
Concomitant administration may increase carbamazepine plasma levels.
Warfarin and other anticoagulants
May increase prothrombin time (INR) and risk of bleeding. Close monitoring is recommended.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and use within the specified period. Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote for cefixime. Hemodialysis or peritoneal dialysis does not remove cefixime from the body in significant amounts. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human breast milk; use with caution in nursing mothers.
