Description
Overview of the medicine
Cefepime is a broad-spectrum, fourth-generation cephalosporin antibiotic used to treat a wide range of serious bacterial infections, including pneumonia, urinary tract infections, skin infections, and febrile neutropenia.
Uses & Indications
Dosage
Adults
Typical adult dosage is 1-2 g intravenously (IV) every 8-12 hours, depending on the severity and type of infection. For complicated urinary tract infections, 500 mg IV or IM every 12 hours.
Elderly
Dosage adjustment is necessary for elderly patients, especially those with renal impairment, based on creatinine clearance.
Renal_impairment
Dose must be adjusted based on creatinine clearance to prevent accumulation and potential neurotoxicity.
How to Take
Cefepime 500 mg injection is typically administered intravenously (IV) over 30 minutes. It can also be given intramuscularly (IM) for mild to moderate urinary tract infections.
Mechanism of Action
Cefepime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily renal, with approximately 85% of the dose excreted unchanged in the urine.
Half life
Approximately 2 hours.
Absorption
Rapid and complete absorption after intravenous (IV) administration. Bioavailability is 100% for IV.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefepime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to any other beta-lactam antibiotic (e.g., penicillin, carbapenems).
Drug Interactions
Probenecid
May decrease the renal clearance of cefepime, leading to increased and prolonged plasma concentrations.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity.
Oral anticoagulants
May enhance the anticoagulant effect.
Loop diuretics (e.g., Furosemide)
May increase the risk of nephrotoxicity with concomitant use.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Reconstituted solutions should be stored at 2°C to 8°C and used within specified timeframes (e.g., 24 hours).
Overdose
In case of overdose, symptoms may include encephalopathy, myoclonus, seizures, and neuromuscular excitability. Management involves symptomatic and supportive treatment. Hemodialysis is effective in removing cefepime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefepime is excreted in human milk; use with caution in nursing mothers.

Lab Monitoring
- Renal function (creatinine, BUN) before and during treatment, especially in elderly and patients with renal impairment.
- Liver function tests (ALT, AST, alkaline phosphatase).
- Complete Blood Count (CBC) during prolonged therapy.
- Prothrombin time/INR for patients on anticoagulants.
Doctor Notes
- Always assess renal function before initiating and throughout therapy, especially in elderly patients and those with pre-existing renal impairment.
- Carefully monitor for neurological adverse events, particularly in patients with impaired renal function who are not appropriately dose-adjusted.
- Consider culture and susceptibility testing to guide therapy, especially in severe or recurrent infections.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve, to prevent antibiotic resistance.
- Report any severe or persistent side effects to your doctor immediately.
- Inform your healthcare provider about all medications you are taking, including over-the-counter drugs and herbal supplements.
- Do not share this medication with others.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered. However, if it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
Cefepime may cause dizziness or confusion in some individuals. Patients should be cautioned about operating machinery or driving until they know how the medication affects them.
Lifestyle Advice
