Description
Overview of the medicine
Cetirizine is a non-sedating antihistamine used for the relief of symptoms associated with seasonal and perennial allergic rhinitis and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
10 mg once daily. Some patients may be adequately controlled by 5 mg once daily.
Elderly
No dose reduction is necessary in elderly patients with normal renal function. For elderly patients with impaired renal function, refer to renal impairment dosage.
Renal_impairment
Mild to moderate (CrCl 30-49 mL/min): 5 mg once daily. Severe (CrCl <30 mL/min): 5 mg every other day. End-stage renal disease/dialysis: Not recommended due to lack of data; use with caution.
How to Take
Oral administration. Can be taken with or without food.
Mechanism of Action
Cetirizine selectively inhibits peripheral H1 receptors, thereby preventing the actions of histamine, such as vasodilation, increased vascular permeability, and itching. It has minimal anticholinergic and antiserotonergic activity.

Onset
Within 20-60 minutes.
Excretion
Primarily via urine (approx. 60% unchanged drug, 10% as metabolites); to a lesser extent via feces.
Half life
Approximately 8 to 11 hours in adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1 hour.
Metabolism
Minimal hepatic metabolism; about two-thirds of the administered dose is excreted unchanged in urine.
Side Effects
Contraindications
- Known hypersensitivity to cetirizine, hydroxyzine, or any piperazine derivative.
- Patients with severe renal impairment (creatinine clearance less than 10 mL/min).
Drug Interactions

CNS depressants (e.g., alcohol, sedatives, hypnotics): Concomitant use may increase CNS depression.

Theophylline: A small decrease in cetirizine clearance has been observed, but is unlikely to be clinically significant.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, tremor, headache, and urinary retention. In case of overdose, symptomatic or supportive treatment should be given. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, it should be used during pregnancy only if clearly needed. Cetirizine is excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.
