Description
Overview of the medicine
Ciclesonide nasal spray is a corticosteroid used to treat symptoms of seasonal and perennial allergic rhinitis, such as sneezing, runny nose, and nasal itching.
Uses & Indications
Dosage
Adults
The recommended dose is 2 sprays per nostril once daily (100 mcg total daily dose). Some patients may achieve full control with 1 spray per nostril twice daily.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
For intranasal use only. Before first use, prime the pump by pressing it several times until a fine mist appears. Shake the bottle gently before each use. Clear your nasal passages, then insert the nozzle into one nostril while closing the other, spray, and inhale gently. Repeat for the other nostril.
Mechanism of Action
Ciclesonide is a prodrug that is converted to an active metabolite (desisobutyryl-ciclesonide) in the nose. This active metabolite is a potent glucocorticoid that exerts anti-inflammatory effects by inhibiting multiple cell types and mediators involved in allergic inflammation.
Pharmacokinetics
Onset
Clinical improvement typically observed within 24-48 hours, with full therapeutic effect often achieved after several days of regular use.
Excretion
Mainly excreted in feces (67%) after hepatic metabolism, with minimal renal excretion.
Half life
The active metabolite, des-ciclesonide, has a terminal elimination half-life of approximately 3-5 hours.
Absorption
Very low systemic bioavailability (<1%) due to extensive first-pass metabolism and limited absorption from the nasal mucosa.
Metabolism
Ciclesonide is a prodrug that is locally activated in the nose by esterases to its active metabolite, des-ciclesonide. It is then primarily metabolized in the liver by CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to ciclesonide or any of the excipients.
- Untreated localized infections involving the nasal mucosa (e.g., fungal, bacterial, viral, or herpetic ocular infections).
Drug Interactions
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Ritonavir)
While ciclesonide is metabolized by CYP3A4, the systemic exposure from nasal administration is very low, making clinically significant interactions unlikely. However, caution is advised with concomitant use.
Storage
Store at controlled room temperature (15°C to 30°C). Do not freeze. Protect from light.
Overdose
Acute overdose is unlikely due to the low systemic bioavailability of ciclesonide. Chronic overuse may lead to systemic corticosteroid effects, such as adrenal suppression. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother. Consult a physician.
