Description
Overview of the medicine
Cinoderm 0.025% Cream is a potent topical corticosteroid containing Clobetasol Propionate. It is used for short-term treatment of severe inflammatory skin conditions that have not responded to less potent corticosteroids.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Treatment should be limited to 2-4 weeks. Total weekly dosage should not exceed 50g.
Elderly
Use with caution. Dosage may need adjustment based on clinical response and skin integrity.
Renal_impairment
No specific dosage adjustment required for topical application, but systemic absorption should be monitored if extensive use is needed.
How to Take
For topical dermatological use only. Apply a thin film to the affected skin area and rub in gently. Do not apply to the face, groin, or axillae unless directed by a physician. Wash hands thoroughly after application.
Mechanism of Action
Clobetasol Propionate is a highly potent corticosteroid. It exerts its effects by binding to glucocorticoid receptors, which leads to the activation of anti-inflammatory genes and the suppression of pro-inflammatory mediators. This results in potent anti-inflammatory, antipruritic, and vasoconstrictive actions.
Pharmacokinetics
Onset
Anti-inflammatory effects typically begin within hours of application.
Excretion
Excreted primarily through the kidneys (urine) and bile.
Half life
Systemic half-life is approximately 3-5 hours, but topical effects can persist longer.
Absorption
Systemic absorption can occur, especially with prolonged use, application to large areas, or under occlusive dressings. Absorption is higher on thinner skin areas or damaged skin.
Metabolism
Primarily metabolized in the liver, similar to other corticosteroids.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any components of the cream.
- Rosacea, acne vulgaris, perioral dermatitis.
- Pruritus without inflammation.
- Perianal and genital pruritus.
- Primary cutaneous viral infections (e.g., herpes simplex, chickenpox).
- Primary bacterial or fungal skin infections.
- Ulcerated skin, wounds.
Drug Interactions
CYP3A4 Inhibitors (e.g., Ritonavir, Itraconazole)
Clobetasol is metabolized by CYP3A4. Concomitant use with strong CYP3A4 inhibitors may increase systemic exposure and risk of systemic adverse effects. This is rare with typical topical use.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely. Chronic overdose or misuse can lead to signs of hypercortisolism (Cushing's syndrome), growth retardation in children, and HPA axis suppression. Treatment involves gradual withdrawal under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Clobetasol Propionate is excreted in human milk; caution should be exercised when administered to a nursing mother. Consult a physician.

Doctor Notes
- Prescribe for the shortest possible duration to achieve desired clinical effect.
- Monitor patients for signs of local and systemic adverse effects, especially in children or when applying to large surface areas.
- Educate patients on proper application technique and potential risks of overuse.
