Overview of the medicine
Ciplon 200 mg Injection contains Ciprofloxacin, a fluoroquinolone antibiotic, used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
200-400 mg IV every 8-12 hours, depending on the severity and type of infection. Infuse over 60 minutes.
Elderly
Dose adjustment may be necessary based on renal function. Generally, the same as adults with normal renal function.
Renal_impairment
Dose reduction is required. For CrCl < 30 mL/min, administer 200-400 mg every 18-24 hours.
How to Take
Administer intravenously over a period of 60 minutes. Do not inject rapidly. Dilution may be required as per specific product instructions.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination. This leads to DNA damage and ultimately bacterial cell death.

Onset
Rapid, typically within 1 hour after IV administration.
Excretion
Primarily excreted renally (50-70% as unchanged drug), with some excretion via bile and feces.
Half life
Approximately 3-5 hours in adults with normal renal function.
Absorption
Rapid and well absorbed after oral administration, but for injection, it achieves high systemic concentrations quickly. Peak plasma levels are reached within 30-60 minutes after IV infusion.
Metabolism
Partially metabolized in the liver (10-20%) to less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones
- Concomitant use with Tizanidine (due to risk of hypotension and somnolence)
- Patients with a history of myasthenia gravis (may exacerbate muscle weakness)
Drug Interactions
Caffeine
May decrease clearance of caffeine and increase caffeine-related side effects.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. Monitor INR.
Tizanidine
Concomitant use increases tizanidine plasma concentrations, leading to hypotension and somnolence. Contraindicated.
Cyclosporine
Increased serum creatinine levels reported with concurrent use. Monitor renal function.
Theophylline
Ciprofloxacin can increase plasma levels of theophylline, leading to increased risk of toxicity (e.g., seizures). Monitor theophylline levels.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Limited data on overdose. Symptoms may include renal impairment, crystalluria, and reversible renal toxicity. Management involves gastric lavage, emesis, or charcoal adsorption, hydration to prevent crystalluria, and supportive care. Hemodialysis or peritoneal dialysis remove only a small amount of ciprofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted into breast milk. A decision should be made whether to discontinue nursing or to discontinue the drug.
