Description
Overview of the medicine
Cipro-C 250 mg Suspension contains Ciprofloxacin, a fluoroquinolone antibiotic, used to treat a wide range of bacterial infections including urinary tract infections, respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted infections.
Uses & Indications
Dosage
Adults
Dosage varies widely based on the type and severity of infection, typically 250-750 mg orally twice daily. For suspension, follow prescribed milliliters.
Elderly
Dosage adjustment may be necessary based on renal function and overall health status.
Renal_impairment
Reduced dosage is recommended for patients with significant renal impairment (creatinine clearance < 30 mL/min).
How to Take
Take orally, with or without food. Do not take with dairy products (milk, yogurt) or calcium-fortified juices, as these can reduce absorption. Shake the suspension well before each use.
Mechanism of Action
Ciprofloxacin acts by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and subsequent bacterial cell death.
Pharmacokinetics
Onset
Within 30 minutes to 2 hours.
Excretion
Primarily excreted renally (50-70% as unchanged drug), with some fecal excretion (15-30%).
Half life
Approximately 3 to 5 hours.
Absorption
Rapid and well-absorbed after oral administration, with peak plasma concentrations occurring within 0.5 to 2 hours.
Metabolism
Undergoes limited hepatic metabolism (10-20%) to several less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- Concurrent administration with tizanidine due to increased risk of adverse effects.
Drug Interactions
Caffeine
Reduced clearance of caffeine, increased stimulant effects.
Warfarin
Potentiation of anticoagulant effect, increased bleeding risk.
Tizanidine
Increased plasma levels of tizanidine, causing hypotension and sedation.
Cyclosporine
Increased nephrotoxicity.
Theophylline
Increased plasma levels of theophylline, leading to toxicity.
Didanosine (DDI)
Reduced absorption of ciprofloxacin due to chelation.
Antacids (containing magnesium, aluminum, calcium)
Decreased absorption of ciprofloxacin.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children. Once reconstituted, store at room temperature or in the refrigerator and discard after 14 days.
Overdose
In case of acute overdose, supportive measures should be instituted. Gastric emptying (e.g., induced vomiting or gastric lavage) may be considered. Adequate hydration should be maintained. Hemodialysis or peritoneal dialysis remove only small amounts of ciprofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Ciprofloxacin has undergone extensive clinical trials demonstrating its efficacy and safety across various bacterial infections. Ongoing research continues to evaluate its role in emerging antimicrobial resistance patterns and specific patient populations.
Lab Monitoring
- Renal function tests (e.g., creatinine, BUN) should be monitored, especially in elderly or renally impaired patients.
- Complete blood count (CBC) if prolonged therapy is anticipated.
- Liver function tests (LFTs) if signs of hepatic dysfunction appear.
Doctor Notes
- Emphasize the black box warnings regarding tendinitis, tendon rupture, peripheral neuropathy, and CNS effects to patients.
- Carefully review patient's medication list for potential drug interactions, especially with warfarin, theophylline, and tizanidine.
- Advise patients about the importance of completing the full course of treatment to prevent resistance.
- Monitor renal function in elderly or renally impaired patients and adjust dosage accordingly.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Avoid exposure to strong sunlight or artificial UV light, as ciprofloxacin can increase photosensitivity.
- Report any new or worsening muscle pain, weakness, numbness, tingling, or joint pain to your doctor immediately.
- Do not take ciprofloxacin with dairy products or calcium-fortified juices. Take at least 2 hours before or 6 hours after.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Ciprofloxacin may cause dizziness or lightheadedness, which could impair your ability to drive or operate machinery. Exercise caution until you know how the medication affects you.
Lifestyle Advice
- Stay well hydrated by drinking plenty of fluids while taking this medication.
- Avoid excessive caffeine intake, as ciprofloxacin can increase caffeine levels in the body.
