Ciprobey 500 mg Tablet is a fluoroquinolone antibiotic used to treat a wide range of bacterial infections including respiratory tract, urinary tract, skin, bone, joint, and gastrointestinal infections.
Uses & Indications
Dosage
Adults
Typically 500 mg orally twice daily (every 12 hours) for 5-14 days depending on the infection. For severe or complicated infections, 750 mg twice daily may be used.
Elderly
Similar to adult dosage, but caution is advised with renal function monitoring and potential for increased risk of adverse effects.
Renal_impairment
Dosage adjustment required based on creatinine clearance. For CrCl 30-60 mL/min: 250-500 mg every 12 hours. For CrCl < 30 mL/min: 250-500 mg every 18-24 hours. For hemodialysis or peritoneal dialysis patients: administer dose after dialysis.
How to Take
Take orally with a full glass of water, with or without food. Do not take with dairy products (milk, yogurt) or calcium-fortified juices alone; take ciprofloxacin at least 2 hours before or 6 hours after these products. Avoid taking with antacids containing magnesium or aluminum, or iron/zinc supplements; take ciprofloxacin at least 2 hours before or 6 hours after these agents.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and ultimately bacterial cell death.

Onset
Varies depending on the type and severity of infection, but symptomatic relief can occur within hours to days.
Excretion
Primarily excreted renally (50-70% as unchanged drug and metabolites) and about 15-30% via feces.
Half life

Absorption

Metabolism
Partially metabolized in the liver via the CYP450 1A2 enzyme system to four active metabolites (desethyleneciprofloxacin, sulfociprofloxacin, oxociprofloxacin, and formylciprofloxacin), although they are less active than the parent drug.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones
- Concomitant use with Tizanidine
Drug Interactions
Warfarin
May increase the anticoagulant effect of warfarin, requiring frequent INR monitoring and potential dose adjustment.
Tizanidine
Concomitant use is contraindicated due to a clinically significant increase in tizanidine plasma concentrations, leading to increased risk of hypotension and somnolence.
Theophylline
Increased plasma levels of theophylline, leading to increased risk of theophylline toxicity (e.g., seizures, arrhythmias). Co-administration should be avoided or theophylline levels carefully monitored.
Corticosteroids

Antacids containing magnesium/aluminum, sucralfate, iron/zinc supplements
May significantly reduce the absorption of ciprofloxacin. Administer ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
In case of acute overdose, gastric lavage may be performed. Symptomatic treatment and supportive measures should be instituted, including adequate hydration to prevent crystalluria. Hemodialysis or peritoneal dialysis remove only small amounts of ciprofloxacin (less than 10% of the drug).
Pregnancy & Lactation
Pregnancy: Ciprofloxacin is classified as Pregnancy Category C. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus, as animal studies have shown potential for arthropathy. Lactation: Ciprofloxacin is excreted into breast milk. Due to the potential for serious adverse reactions in nursing infants (e.g., permanent cartilage damage), a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
