Cipronaaf (Ciprofloxacin) is a broad-spectrum fluoroquinolone antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
250-750 mg orally twice daily, depending on the infection severity and type.
Elderly
Similar to adults, but dosage adjustment may be required based on renal function.
Renal_impairment
Dosage must be adjusted in patients with creatinine clearance < 50 mL/min. For example, half the usual dose or increased dosing interval.
How to Take
Cipronaaf (Ciprofloxacin) tablets can be taken with or without food. Avoid taking with dairy products (milk, yogurt) or calcium-fortified juices, as these can reduce absorption. Take with plenty of fluids.
Mechanism of Action
Cipronaaf (Ciprofloxacin) inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Rapid, typically within 1-2 hours of oral administration.
Excretion
Primarily excreted renally (50-70% as unchanged drug) and to a lesser extent via feces (15%).
Half life
Approximately 3-5 hours.
Absorption
Well absorbed orally, with an absolute bioavailability of about 70-80%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Partially metabolized in the liver via the CYP450 system to less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to Ciprofloxacin or other fluoroquinolones
- Concomitant administration with Tizanidine
Drug Interactions
Warfarin
May enhance the anticoagulant effect of Warfarin, increasing bleeding risk.
Theophylline
Increased plasma concentrations of Theophylline, potentially leading to toxicity.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
Antacids containing magnesium/aluminum, Sucralfate, Zinc/Iron supplements
Reduced absorption of Cipronaaf (Ciprofloxacin) if taken concomitantly. Administer Cipronaaf (Ciprofloxacin) 2-4 hours before or 4-6 hours after these agents.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be considered. Symptomatic and supportive treatment should be initiated. Adequate hydration is important. Hemodialysis or peritoneal dialysis remove only small amounts of ciprofloxacin.
Pregnancy & Lactation
Cipronaaf (Ciprofloxacin) is generally not recommended during pregnancy or lactation due to potential risks to the fetus/infant, including cartilage damage. Use only if the potential benefit outweighs the potential risk. Ciprofloxacin is excreted into breast milk.
