How to Take
Take orally with or without food. Do not take with dairy products or calcium-fortified juices. Shake the bottle well before each use.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination.
Pharmacokinetics
Onset
Variable, depends on the type and severity of infection.
Excretion
Primarily via urine (renal) as unchanged drug and metabolites; some fecal excretion also occurs.
Half life
3-5 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract. Bioavailability is approximately 70-80%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Partially metabolized in the liver to less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other quinolones.
- •Concomitant use with Tizanidine.
- •Children and adolescents (due to risk of cartilage damage, unless benefits outweigh risks).
Drug Interactions
Caffeine
May decrease caffeine clearance, leading to increased caffeine levels.
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Tizanidine
Contraindicated; Ciprofloxacin significantly increases tizanidine plasma concentrations.
Theophylline
May increase theophylline plasma levels, leading to increased risk of toxicity.
Antacids (containing Mg/Al), Iron, Zinc, Sucralfate
Reduce ciprofloxacin absorption when co-administered. Administer ciprofloxacin 2 hours before or 6 hours after these agents.
Storage
Store at room temperature (below 30°C), protected from light and moisture. Do not freeze.
Overdose
In case of overdose, gastric lavage may be performed. Symptomatic and supportive treatment should be initiated. There is no specific antidote.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Ciprofloxacin is excreted into breast milk and may cause arthropathy in nursing infants; use with caution or consider alternatives.
