Ciprofloxacin is a broad-spectrum antibiotic from the fluoroquinolone class, used to treat various bacterial infections including those affecting the urinary tract, respiratory tract, skin, and gastrointestinal system.
Uses & Indications
Dosage
Adults
Dosage varies depending on the type and severity of infection, typically 250 mg to 750 mg orally twice daily. For suspension, this usually translates to 10 mL to 30 mL of 250 mg/5mL suspension twice daily.
Elderly
Similar to adult dosage, but dose adjustment may be necessary based on renal function.
Renal_impairment
Dose adjustment is required for patients with creatinine clearance < 50 mL/min. Consult product leaflet for specific guidelines.
How to Take
Take orally, with or without food. Shake the bottle well before each use. Do not take with dairy products (milk, yogurt) or calcium-fortified juices. Avoid taking with antacids containing magnesium or aluminum; separate doses by at least 2 hours before or 6 hours after such products.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.

Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other quinolone antibiotics.
- •Concomitant administration with tizanidine.
Drug Interactions
NSAIDs
Concomitant use with NSAIDs may increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May increase the anticoagulant effect, requiring close monitoring of INR.
Tizanidine
Concomitant use is contraindicated due to increased risk of hypotension and sedation.
Cyclosporine
May increase serum creatinine levels.
Theophylline
May increase plasma concentrations of theophylline, potentially leading to toxicity.
Antacids/Iron/Zinc/Calcium
Products containing di- or trivalent cations can significantly reduce ciprofloxacin absorption; administer Ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children. Do not freeze the reconstituted suspension.
Overdose
In case of overdose, gastric lavage and activated charcoal may be considered. Maintain adequate hydration to prevent crystalluria. Symptomatic and supportive care should be provided.
Pregnancy & Lactation
Pregnancy Category C. Ciprofloxacin should generally be avoided during pregnancy due to potential for arthropathy in immature animals, though human data is limited. It is excreted into breast milk; use with caution or consider an alternative during lactation.
