Overview of the medicine
Ciprox-XR 1000 mg Tablet is an extended-release fluoroquinolone antibiotic used to treat various bacterial infections, including urinary tract infections, chronic bacterial prostatitis, and complicated intra-abdominal infections.
Uses & Indications
Dosage
Adults
For cUTI or AP: 1000 mg orally once daily for 7-14 days. For Chronic Bacterial Prostatitis: 1000 mg orally once daily for 28 days.
Elderly
No specific dose adjustment typically required based on age alone, but renal function should be assessed.
Renal_impairment
Adjust dose for severe renal impairment (CrCl < 30 mL/min). For cUTI/AP, consider 500 mg XR every 24 hours. Refer to specific guidelines.
How to Take
Take orally with a glass of water, preferably at the same time each day. Do not crush, cut, or chew the tablet. Can be taken with or without food, but avoid taking with dairy products or calcium-fortified juices alone.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Onset of action within 1-2 hours, with peak plasma concentrations occurring 1-4 hours post-dose for XR.
Excretion
Primarily via urine (50-70% as unchanged drug) and to a lesser extent via feces.
Half life
Approximately 3-5 hours.
Absorption
Well absorbed orally. Extended-release formulation provides sustained plasma concentrations.
Metabolism
Partially metabolized in the liver (15-30%) to less active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to ciprofloxacin or other fluoroquinolones
- •Concomitant use with tizanidine
Drug Interactions
Warfarin
Increased anticoagulant effect, requiring INR monitoring.
Tizanidine
Increased risk of hypotension and sedation due to increased tizanidine plasma concentrations. Concomitant use is contraindicated.
Theophylline
Increased plasma levels of theophylline, leading to toxicity. Monitor theophylline levels.
Corticosteroids
Increased risk of tendon rupture.
Antacids (containing Mg/Al), Sucralfate, Didanosine, Iron/Zinc salts
Reduced absorption of ciprofloxacin. Administer ciprofloxacin 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include renal toxicity, crystalluria, CNS effects (confusion, dizziness, tremor, seizures). Management involves gastric emptying (emesis or gastric lavage) and symptomatic and supportive treatment, including maintaining adequate hydration to prevent crystalluria.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Ciprofloxacin is excreted into breast milk. Discontinue nursing or the drug, considering the importance of the drug to the mother.
