Overview of the medicine
Ciprozen 250 mg Suspension is a broad-spectrum fluoroquinolone antibiotic used to treat a variety of bacterial infections, including urinary tract infections, respiratory tract infections, skin and soft tissue infections, and certain gastrointestinal infections.
Uses & Indications
Dosage
Adults
Dosage varies greatly depending on the infection type and severity. Typical range for suspension is 250-750 mg orally twice daily. For specific infections, follow physician's instructions.
Elderly
No specific dose adjustment for elderly patients with normal renal function. For impaired renal function, refer to renal impairment dosage.
Renal_impairment
For creatinine clearance 30-60 mL/min: give 250-500 mg every 12 hours. For creatinine clearance <30 mL/min: give 250-500 mg every 18-24 hours.
How to Take
Shake the suspension well before each use. Can be taken with or without food. Avoid taking with dairy products, calcium-fortified juices, or antacids, as these can reduce absorption. Take at least 2 hours before or 6 hours after such products.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and ultimately bacterial cell death.

Within 1-2 hours
Excretion
Primarily excreted unchanged in the urine (50-70%) and feces (20-35%).
Half life
Approximately 4 hours (range 3-5 hours), may be prolonged in renal impairment.
Absorption
Rapidly and well absorbed from the gastrointestinal tract (bioavailability 70-80%). Food may delay absorption but does not significantly alter overall bioavailability.
Metabolism
Partially metabolized in the liver to four active metabolites, but the majority is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones
- Concomitant administration with tizanidine (risk of hypotension and somnolence)
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. Monitor INR/PT.
Tizanidine
Concomitant use is contraindicated due to increased risk of hypotension and somnolence.
Methotrexate
May increase methotrexate plasma levels, potentially leading to toxicity. Monitor methotrexate levels.
Theophylline
Ciprofloxacin can increase theophylline plasma concentrations, potentially leading to toxicity. Monitor levels and adjust theophylline dose.
Antacids (containing magnesium, aluminum, calcium), Sucralfate, Iron/Zinc salts
Can significantly reduce ciprofloxacin absorption. Take ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store in a cool, dry place, below 30°C. Protect from light. Keep out of reach of children. Do not freeze the reconstituted suspension.
Overdose
In case of acute overdose, reversible renal toxicity has been reported. Gastric lavage may be performed. The patient should be carefully observed and given supportive treatment, including maintaining adequate hydration. Hemodialysis or peritoneal dialysis remove only small amounts of ciprofloxacin.
Pregnancy & Lactation
Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk and may cause adverse effects in a nursing infant. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
