Cistor 1 mg Injection is a potent non-opioid analgesic and anti-inflammatory agent used for the short-term management of acute moderate to severe pain in adults. It is administered parenterally for rapid pain relief.
Uses & Indications
Dosage
Adults
Initial dose: 1 mg intravenously (IV) slowly over 15 minutes or intramuscularly (IM) deep into a large muscle. Subsequent doses: 0.5-1 mg every 6-8 hours as needed. Maximum daily dose: 3 mg.
Elderly
Initial dose reduction may be considered (e.g., 0.5 mg every 8-12 hours) due to potential decreased renal function and increased sensitivity. Maximum daily dose: 2 mg.
Renal_impairment
Adjust dose based on creatinine clearance. For CrCl 30-60 mL/min, reduce dose by 50%. Avoid use in severe renal impairment (CrCl <30 mL/min).
How to Take
Cistor 1 mg Injection should be administered by a healthcare professional. For intravenous administration, inject slowly over at least 15 minutes into a vein. For intramuscular administration, inject deep into a large muscle mass (e.g., gluteal muscle). Do not administer intrathecally or epidurally.
Mechanism of Action
Cistor acts by inhibiting cyclooxygenase (COX) enzymes, primarily COX-2, leading to reduced prostaglandin synthesis. This results in decreased pain perception, inflammation, and fever.

Excretion
Mainly renal excretion of metabolites (approximately 60-70%) and a small percentage of unchanged drug (10-20%). Some excretion occurs via bile/feces.

Rapid and complete systemic absorption upon intravenous (IV) administration. Good absorption from intramuscular (IM) site, with bioavailability approaching 90-100%.
Metabolism

Side Effects
Contraindications

Drug Interactions
Lithium
May increase plasma lithium levels, leading to toxicity.
Methotrexate
May increase methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
ACE inhibitors and ARBs
May reduce the antihypertensive effect and increase the risk of renal impairment.
Diuretics (e.g., furosemide)
May reduce natriuretic and diuretic effects, and increase the risk of renal toxicity.
Warfarin and other anticoagulants
Increased risk of bleeding.

Overdose
Symptoms of overdose may include severe nausea, vomiting, abdominal pain, lethargy, drowsiness, blurred vision, dizziness, and in severe cases, renal failure or respiratory depression. Management is primarily supportive; maintain airway, monitor vital signs, and provide symptomatic treatment. Hemodialysis is unlikely to be effective.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy is not recommended, especially during the third trimester, due to potential adverse effects on fetal cardiovascular system and inhibition of labor. Small amounts may pass into breast milk; use with caution in lactating mothers after consulting a doctor and weighing benefits against risks.
