Description
Overview of the medicine
Citritol is a urinary alkalinizer containing a combination of potassium citrate, sodium citrate, and citric acid. It works by making the urine less acidic, which helps to relieve discomfort associated with urinary tract infections and to prevent the formation of certain kidney stones.
Uses & Indications
Dosage
Adults
Typically, 1-2 sachets (or 10-15 ml solution) diluted in water, 2-3 times daily, or as directed by physician. Adjust to maintain urinary pH between 6.0-7.0.
Elderly
Similar to adult dosage, but caution with renal impairment.
Renal_impairment
Use with caution; dose adjustment may be necessary or contraindicated in severe cases.
How to Take
Dissolve the granules or dilute the solution in a glass of water and drink immediately. Take after meals to minimize gastrointestinal upset.
Mechanism of Action
The citrate ions in Citritol are metabolized to bicarbonate, which increases the urinary pH, making the urine less acidic (alkaline). This alkalinization helps to alleviate dysuria (painful urination) associated with UTIs and prevents the crystallization of uric acid and cystine, thus preventing stone formation.
Pharmacokinetics
Onset
Within 30-60 minutes after administration
Excretion
Mainly renal as bicarbonate and unchanged citrate.
Half life
Short (citrate is rapidly metabolized)
Absorption
Well absorbed orally, citrate is readily metabolized.
Metabolism
Citrate is metabolized in the liver to bicarbonate.
Side Effects
Contraindications
- Severe renal impairment (anuria, oliguria, azotemia)
- Untreated Addison's disease
- Acute dehydration
- Hyperkalemia
- Concomitant use with potassium-sparing diuretics
- Gastric ulcer (caution)
Drug Interactions
ACE inhibitors, ARBs
Increased risk of hyperkalemia
Potassium-sparing diuretics
Increased risk of hyperkalemia
Aluminum-containing antacids
Increased absorption of aluminum
Urinary alkalinization may affect excretion of other drugs (e.g., increased excretion of acidic drugs, decreased excretion of basic drugs).
Altered excretion of other drugs
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hyperkalemia, metabolic alkalosis, and gastrointestinal upset. Management involves discontinuing the drug, correcting electrolyte imbalance, and symptomatic treatment.
Pregnancy & Lactation
Generally considered safe, but use with caution and under medical advice. Consult a doctor before use during pregnancy or lactation.

- Monitor serum potassium, especially in elderly and renally impaired patients.
- Counsel patients on correct dilution and administration.
Patient Guidelines
- Take with a full glass of water.
- Do not chew or crush granules.
- Report any unusual side effects to your doctor.
- Maintain adequate fluid intake.
