Description
Overview of the medicine
Clecof-100-mg syrup is a combination medication used to relieve cough and chest congestion caused by the common cold, bronchitis, and other breathing illnesses. It works by loosening mucus in the lungs and making it easier to cough up, while also suppressing the cough reflex.
Uses & Indications
Dosage
Adults
Adults and children 12 years and older: 10 mL (2 teaspoonfuls) orally every 4 hours, not to exceed 60 mL in 24 hours. Children 6 to under 12 years: 5 mL (1 teaspoonful) every 4 hours, not to exceed 30 mL in 24 hours. Children 2 to under 6 years: 2.5 mL (1/2 teaspoonful) every 4 hours, not to exceed 15 mL in 24 hours. Not for children under 2 years.
Elderly
No specific dosage adjustment needed, but caution advised for those with compromised renal or hepatic function. Always consult a physician.
Renal_impairment
Use with caution. Dosage reduction may be necessary in severe renal impairment. Consult a physician.
How to Take
Take orally, measure with a dosing spoon or cup. Do not dilute. May be taken with or without food. Drink plenty of fluids to help loosen congestion.
Mechanism of Action
Dextromethorphan acts on the cough center in the medulla oblongata to elevate the cough threshold, thereby suppressing the cough reflex. Guaifenesin is an expectorant that works by increasing the volume and decreasing the viscosity of secretions in the trachea and bronchi, thereby making it easier to remove by coughing.
Pharmacokinetics
Onset
Dextromethorphan: 15-30 minutes; Guaifenesin: ~30 minutes
Excretion
Mainly renal (urine) as metabolites.
Half life
Dextromethorphan: 2-4 hours (extensive metabolizers), up to 45 hours (poor metabolizers); Guaifenesin: ~1 hour
Absorption
Rapidly absorbed from the gastrointestinal tract. Dextromethorphan undergoes rapid and extensive first-pass metabolism.
Metabolism
Dextromethorphan: Hepatic, primarily via CYP2D6 to dextrorphan (active metabolite); Guaifenesin: Hepatic, to beta-(2-methoxyphenoxy)lactic acid.
Side Effects

Alcohol/CNS Depressants
May enhance CNS depressant effects of dextromethorphan.
