Description
Overview of the medicine
Cleven-6000 is a low molecular weight heparin (LMWH) used to prevent and treat blood clots (thrombosis). It contains enoxaparin sodium as its active ingredient.
Uses & Indications
Dosage
Adults
DVT prophylaxis (surgical): 20 mg or 40 mg SC once daily. DVT/PE treatment: 1 mg/kg SC every 12 hours or 1.5 mg/kg SC once daily. Unstable angina/NSTEMI: 1 mg/kg SC every 12 hours.
Elderly
No dosage adjustment is generally required, but close monitoring of renal function is recommended.
Renal_impairment
Severe renal impairment (CrCl < 30 mL/min): Dose reduction is necessary. For treatment, 1 mg/kg SC once daily. For prophylaxis, 30 mg SC once daily.
How to Take
Administer by deep subcutaneous injection, preferably into the abdominal wall (alternating left and right sides) or the outer thigh. Do not inject intramuscularly. Do not mix with other injections.
Mechanism of Action
Enoxaparin binds to antithrombin III, which then inactivates Factor Xa (and to a lesser extent, Factor IIa/thrombin), preventing the formation and growth of blood clots.
Pharmacokinetics
Onset
Peak anti-Xa activity achieved within 3-5 hours after subcutaneous administration.
Excretion
Primarily excreted renally, with approximately 10% of the dose excreted as active fragments in urine.
Half life
Anti-Xa elimination half-life is approximately 4-5 hours after a single subcutaneous dose, and up to 7 hours after repeated dosing.
Absorption
Approximately 100% bioavailability after subcutaneous injection.
Metabolism
Primarily metabolized in the liver by desulfation and depolymerization into lower molecular weight species with reduced anticoagulant activity.
Side Effects
Contraindications
- Hypersensitivity to enoxaparin, heparin, or porcine products.
- Active major bleeding or conditions with a high risk of uncontrolled bleeding.
- History of heparin-induced thrombocytopenia (HIT) with or without thrombosis.
Drug Interactions
Other thrombolytic agents
Significantly increased risk of hemorrhage.
NSAIDs (e.g., Ibuprofen, Diclofenac)
Increased risk of bleeding. Use with caution.
Oral anticoagulants (e.g., Warfarin)
Increased risk of bleeding. Close monitoring and careful transition are required.
Antiplatelet agents (e.g., Aspirin, Clopidogrel)
Increased risk of bleeding. Concomitant use should be with caution.
Storage
Store below 25°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Overdose may lead to hemorrhagic complications. The anticoagulant effects can be largely neutralized by the slow intravenous injection of protamine sulfate. However, protamine sulfate only partially neutralizes anti-Xa activity.
Pregnancy & Lactation
Pregnancy Category B. Enoxaparin does not cross the placental barrier. Use in pregnancy only if clearly needed after careful risk-benefit assessment. It is unlikely to be excreted into breast milk.
