Overview of the medicine
Clindamet 150 mg Capsule is an antibiotic used to treat a variety of bacterial infections, including those of the skin, soft tissue, respiratory tract, and female reproductive organs. It works by stopping the growth of susceptible bacteria.
Uses & Indications
Dosage
Adults
For serious infections: 150-300 mg every 6-8 hours. For more severe infections: 300-450 mg every 6-8 hours.
Elderly
No specific dose adjustment required solely based on age; however, monitor renal and hepatic function, especially if impaired.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe renal impairment, consider extending the dosing interval to every 8-12 hours.
How to Take
Take the capsule orally with a full glass of water, with or without food. To prevent esophageal irritation, it is important to take Clindamet in an upright position and avoid lying down for at least 30 minutes after ingestion.
Mechanism of Action
Clindamycin binds to the 50S ribosomal subunit of susceptible bacteria, inhibiting bacterial protein synthesis. This results in bacteriostatic effects, preventing bacterial growth and replication.
Pharmacokinetics
Onset
Antibacterial effect typically begins within 1 hour after oral administration.
Excretion
About 10-20% excreted in urine, 4% in feces, and the remainder as inactive metabolites. Elimination occurs primarily through bile and urine.
Half life
Approximately 2-3 hours in adults with normal renal and hepatic function. This half-life may be prolonged in patients with severe renal or hepatic impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract (about 90% bioavailable) after oral administration. Peak plasma concentrations are reached within 45-60 minutes.
Metabolism
Extensively metabolized in the liver to active (N-demethyl clindamycin) and inactive metabolites. CYP3A4 is the primary enzyme involved.
Side Effects
Contraindications
- •Hypersensitivity to Clindamycin, Lincomycin, or any component of the formulation.
- •Patients with a history of Clindamycin-associated colitis.
Drug Interactions
Erythromycin
Antagonism between clindamycin and erythromycin has been demonstrated in vitro; avoid concurrent use.
Oral contraceptives
May reduce the efficacy of oral contraceptives; advise patients to use an alternative or additional method of contraception.
Neuromuscular blocking agents
Clindamycin may enhance the neuromuscular blocking effects, leading to increased muscle weakness.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
No specific antidote for clindamycin overdose. Management is symptomatic and supportive, including maintaining airway, breathing, and circulation. Hemodialysis and peritoneal dialysis are not effective in removing clindamycin from the blood.
Pregnancy & Lactation
Pregnancy Category B. Studies in pregnant animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Clindamycin should be used during pregnancy only if clearly needed. It is excreted in breast milk; caution should be exercised when administered to a nursing woman.
