Description
Overview of the medicine
Clix 125 mg Suspension is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections, including those of the respiratory tract, urinary tract, and middle ear. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Not typically formulated for adult primary use as 125 mg suspension. Adults usually take 400 mg once daily (e.g., as tablets).
Elderly
No specific dosage adjustment for elderly unless renal function is impaired. Refer to renal impairment guidelines.
Pediatric
Children 6 months to 12 years: 8 mg/kg/day orally as a single dose or in two divided doses. For 125 mg suspension, this would typically mean 2.5 ml for every 5-6 kg body weight, or as directed by physician.
Renal_impairment
For creatinine clearance < 30 mL/min, the dose should be reduced to 200 mg daily. For patients on chronic ambulatory peritoneal dialysis or hemodialysis, similar dose reduction is recommended.
How to Take
Administer orally, with or without food. Shake the suspension well before each use. Use the provided measuring device (spoon or syringe) to ensure accurate dosing. Do not use a household spoon.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which are responsible for the final transpeptidation step of peptidoglycan synthesis. This inhibition leads to the disruption of the cell wall, ultimately causing bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Primarily excreted unchanged in urine via glomerular filtration and tubular secretion. A small portion is excreted in bile.
Half life
About 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food slows absorption but does not significantly affect the total absorbed amount.
Metabolism
Not extensively metabolized. Some metabolites may be formed.
Side Effects
Contraindications
- Hypersensitivity to cefixime, other cephalosporins, or any component of the formulation
- Patients with a history of severe hypersensitivity reactions to penicillin (due to potential cross-reactivity)
Drug Interactions
Nifedipine
May increase cefixime concentrations.
Carbamazepine
Elevated carbamazepine levels may occur. Monitor carbamazepine levels.
Oral Contraceptives
May reduce efficacy of oral contraceptives (though clinical significance is debatable, advise additional contraceptive methods).
Warfarin and other anticoagulants
Increased prothrombin time (PT) and INR, leading to increased bleeding risk. Close monitoring of coagulation parameters is recommended.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store refrigerated (2-8°C) and do not freeze. Keep out of reach of children.
Overdose
Gastric lavage may be indicated; otherwise, no specific antidote exists. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Renal function tests (e.g., serum creatinine, BUN) in patients with pre-existing renal impairment or during prolonged high-dose therapy.
- Liver function tests (e.g., AST, ALT) if abnormalities are suspected.
- Prothrombin time (PT) / INR for patients on concomitant anticoagulants.
Doctor Notes
- Confirm bacterial infection before prescribing, as cefixime is ineffective against viral infections.
- Counsel patients on completing the full course to prevent resistance and recurrence.
- Monitor for signs of C. difficile-associated diarrhea, which can occur even weeks after therapy.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Shake the suspension well before each use and use the provided measuring device.
- Do not save suspension for future use; discard any unused portion after the specified period (usually 7-14 days after reconstitution).
- Report any severe or persistent side effects to your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Cefixime generally does not impair the ability to drive or operate machinery. However, if you experience dizziness or headache, avoid these activities until you feel well.
Lifestyle Advice
- Maintain good hydration.
- Avoid self-medication with antibiotics to prevent antibiotic resistance.
- Inform healthcare provider about all current medications and medical conditions.
