Description
Overview of the medicine
Clobederm 0.05% Ointment is a potent corticosteroid for topical use. It is used to treat various inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Treatment should be limited to 2 weeks for continuous use and no more than 50 grams per week. Re-evaluate if no improvement after 2 weeks.
Elderly
Use with caution due to increased risk of systemic side effects and skin atrophy. Lower frequency or shorter duration may be considered.
Renal_impairment
No specific dosage adjustment required for renal impairment for topical use, but monitor for systemic absorption with extensive use.
How to Take
For external use only. Apply a thin film to the affected skin area. Rub in gently and completely. Wash hands thoroughly after application (unless hands are the treated area). Do not apply to the face, groin, or axillae unless directed by a physician. Avoid contact with eyes.
Mechanism of Action
Clobetasol propionate is a highly potent corticosteroid that exhibits anti-inflammatory, antipruritic, and vasoconstrictive actions. It works by inhibiting the release of mediators involved in inflammation and allergic reactions, and by suppressing the migration of various cells to the site of inflammation.
Pharmacokinetics
Onset
Anti-inflammatory effects typically begin within a few hours of application.
Excretion
Excreted mainly by the kidneys, with some biliary excretion.
Half life
Plasma half-life of absorbed Clobetasol Propionate is approximately 3-5 hours, but local effects persist longer.
Absorption
Systemic absorption of topical corticosteroids can occur, especially with prolonged use, extensive surface area application, occlusion, or on damaged skin. Absorption varies widely.
Metabolism
Primarily metabolized in the liver, similar to other corticosteroids, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any component of the formulation
- Bacterial, fungal, or viral infections of the skin (untreated)
- Rosacea
- Perioral dermatitis
- Acne vulgaris
- Pruritus without inflammation
Drug Interactions
Immunosuppressants
Concurrent use may increase the risk of systemic infections.
CYP3A4 Inhibitors (e.g., Ritonavir, Itraconazole)
May increase systemic exposure to Clobetasol Propionate, leading to increased risk of systemic side effects.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose with topical corticosteroids is unlikely. However, chronic overdose or misuse can lead to systemic effects such as HPA axis suppression, Cushing's syndrome, and other steroid-related adverse effects. Treatment is symptomatic and supportive. Gradually withdraw the drug under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether topical administration of corticosteroids can result in sufficient systemic absorption to produce detectable quantities in breast milk. Use with caution in nursing mothers.

Clinical Trials
Numerous clinical trials have established the efficacy and safety of clobetasol propionate for various dermatological conditions. Studies typically focus on comparative efficacy against other steroids or placebos, and assessment of local and systemic side effects.
Lab Monitoring
- Periodic evaluation for HPA axis suppression (e.g., ACTH stimulation test, morning plasma cortisol) for patients using large amounts, over large surface areas, or for prolonged periods.
- Blood glucose levels (especially in diabetic patients)
Doctor Notes
- Counsel patients on proper application technique and duration limits to minimize systemic absorption and local side effects.
- Monitor for signs of HPA axis suppression, especially in pediatric patients or those using large amounts under occlusion.
- Avoid abrupt discontinuation after prolonged use; gradually reduce frequency if needed to prevent rebound flare-ups.
Patient Guidelines
- Use exactly as directed by your doctor. Do not exceed the prescribed dose or duration of treatment.
- Do not use for longer than 2 weeks, or more than 50 grams per week, unless advised by your doctor.
- Avoid application to the face, groin, or underarms, and avoid contact with eyes.
- Do not cover the treated area with bandages or other occlusive dressings unless specifically instructed by your doctor.
Missed Dose Advice
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply extra medicine to make up for a missed dose.
Driving Precautions
Clobederm 0.05% Ointment is for topical use and is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Keep the affected skin clean and moisturized (with non-medicated moisturizers).
- Avoid irritants and allergens that may worsen your skin condition.
