Overview of the medicine
Clobid contains Clopidogrel, an antiplatelet agent used to prevent blood clots in patients with cardiovascular disease, recent heart attack, stroke, or peripheral arterial disease.
Uses & Indications
Dosage
Adults
For ACS (STEMI/NSTEMI/UA): Loading dose of 300 mg or 600 mg, followed by 75 mg once daily. For MI/stroke/PAD: 75 mg once daily.
Elderly
No specific dose adjustment is generally needed based on age alone.
Renal_impairment
No dose adjustment generally required for mild to moderate renal impairment. Caution advised in severe impairment due to limited data.
How to Take
Administer orally, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Clopidogrel selectively inhibits the binding of adenosine diphosphate (ADP) to its platelet receptor (P2Y12), thereby inhibiting ADP-mediated platelet activation and aggregation.
Pharmacokinetics
Onset
Antiplatelet effect begins within 2 hours of a single dose, maximal effect observed in 3-7 days with repeated dosing.
Excretion
Approximately 50% via urine and 46% via feces within 5 days of administration.
Half life
Parent compound has a half-life of approximately 6 hours. The active metabolite has a very short half-life (< 1 hour).
Absorption
Rapidly absorbed after oral administration, bioavailability is about 50% for the parent drug.
Metabolism
Extensively metabolized in the liver by CYP450 enzymes (primarily CYP2C19) to an active thiol metabolite and inactive carboxylic acid derivatives.

Overdose

Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but human data are limited. Use only if clearly needed. It is unknown if Clopidogrel is excreted in human milk; use during lactation is generally not recommended due to potential for adverse effects on the infant.
