Description
Overview of the medicine
Diclofenac 1% w/w gel is a topical non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain and inflammation in muscles, joints, tendons, and ligaments.
Uses & Indications
Dosage
Adults
Apply 2-4 grams of gel (a quantity the size of a cherry or walnut) to the affected area 3-4 times daily and rub in gently.
Elderly
No specific dose adjustment is required, apply as for adults.
Renal_impairment
No dose adjustment needed for topical application due to minimal systemic absorption.
How to Take
Apply a small amount of gel to the skin over the affected area and rub in gently. Wash hands thoroughly after application.
Mechanism of Action
It works by inhibiting the activity of cyclooxygenase (COX-1 and COX-2) enzymes, which are involved in the synthesis of prostaglandins, thus reducing pain and inflammation.
Pharmacokinetics
Onset
Pain relief can begin within 30-60 minutes after application.
Excretion
Mainly renal excretion for systemically absorbed portion; negligible with topical application.
Half life
Systemic half-life is approximately 1-2 hours, but local therapeutic effects persist longer.
Absorption
Limited systemic absorption, high local concentration in affected tissues.
Metabolism
Minimal hepatic metabolism due to limited systemic absorption with topical use.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, other NSAIDs, or any component of the gel.
- Application to open wounds, infected skin, or broken skin.
- Third trimester of pregnancy.
Drug Interactions
Systemic NSAIDs
Increased risk of systemic side effects if used concurrently, though absorption is minimal.
Warfarin and other anticoagulants
Possible increased risk of bleeding, although systemic exposure is low.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose is unlikely with topical application. Excessive application over large skin areas may lead to systemic effects. In case of suspected overdose, wipe off excess gel and provide symptomatic and supportive treatment.
Pregnancy & Lactation
Use with caution during pregnancy, especially in the first two trimesters. Contraindicated in the third trimester. Consult a doctor before use during lactation, as diclofenac may pass into breast milk in small amounts.

Numerous clinical trials have demonstrated the efficacy of topical diclofenac in relieving pain and improving function in various localized musculoskeletal conditions, including osteoarthritis.
Lab Monitoring
- Routine laboratory monitoring is generally not required for topical diclofenac application due to minimal systemic absorption.
Doctor Notes
- Advise patients on the correct application technique and precautions (e.g., avoiding eyes, mucous membranes, broken skin).
- Educate patients about the minimal systemic absorption and lower risk of systemic side effects compared to oral NSAIDs.
- Caution against concomitant use with systemic NSAIDs to avoid potential additive side effects.
Patient Guidelines
- Avoid contact with eyes, mouth, or other mucous membranes.
- Do not apply to open wounds, infected skin, or damaged skin.
- Do not cover the treated area with an occlusive bandage.
- Do not use for more than 7 days without consulting a doctor.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule.
Driving Precautions
Topical diclofenac gel is unlikely to affect the ability to drive or operate machinery, as systemic absorption is minimal.
Lifestyle Advice
- For acute injuries, consider RICE (Rest, Ice, Compression, Elevation) therapy in conjunction with the gel.
- Maintain proper posture and ergonomics to prevent recurrent pain.
