Description
Overview of the medicine
Clont (Pentoxifylline) is a hemorheologic agent used to treat symptoms of intermittent claudication due to chronic occlusive arterial disease. It works by reducing blood viscosity and improving erythrocyte flexibility.
Uses & Indications
Dosage
Adults
Typically 400 mg three times daily with meals. May reduce to 400 mg twice daily if gastrointestinal side effects occur.
Elderly
No specific dose adjustment required, but monitor for adverse effects.
Renal_impairment
Dose reduction may be necessary in severe renal impairment (e.g., creatinine clearance <30 mL/min).
How to Take
Take with meals to minimize gastrointestinal upset. Swallow tablets whole; do not crush, chew, or break.
Mechanism of Action
Pentoxifylline improves blood flow by decreasing blood viscosity and increasing erythrocyte flexibility, thus enhancing tissue oxygenation. It may also have anti-inflammatory and anti-thrombotic properties.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 1-2 hours for immediate release, slower for sustained release.
Excretion
Primarily via urine (95%), mainly as metabolites.
Half life
Approximately 0.4-0.8 hours for the parent drug; 1.0-1.6 hours for active metabolites.
Absorption
Well absorbed orally, but undergoes extensive first-pass metabolism.
Metabolism
Extensive hepatic first-pass metabolism to active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to pentoxifylline, other methylxanthines (e.g., caffeine, theophylline).
- •Recent cerebral or retinal hemorrhage.
- •Severe coronary artery disease with hypotension.
Drug Interactions
Theophylline
May increase theophylline levels and toxicity.
Antihypertensives
May potentiate hypotensive effects.
Anticoagulants (e.g., Warfarin)
May increase risk of bleeding; monitor INR.
Antiplatelet agents (e.g., Aspirin, Clopidogrel)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include flushing, hypotension, convulsions, fever, agitation, somnolence, and loss of consciousness. Treatment is supportive and symptomatic; gastric lavage may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Excreted in breast milk, use with caution in lactating women.
