Description
Overview of the medicine
Closon 0.05% w/w Cream contains clobetasol propionate, a very potent topical corticosteroid. It is used for the short-term treatment of inflammatory and pruritic manifestations of moderate to severe corticosteroid-responsive dermatoses, such as psoriasis, recalcitrant eczemas, lichen planus, discoid lupus erythematosus, and other conditions that have not responded satisfactorily to less potent steroids.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily for up to 4 weeks. Do not use for more than 4 weeks; if further treatment is needed, consider a less potent corticosteroid. Max dose 50g per week.
Elderly
Same as adults, but use with caution due to thinner skin and increased potential for systemic absorption.
Renal_impairment
No specific dosage adjustment recommended for topical use; however, use with caution if extensive skin involvement or prolonged treatment due to potential systemic absorption.
How to Take
For external use only. Apply a thin layer to the affected skin area and rub in gently. Wash hands immediately after application, unless hands are the treated area. Do not cover the treated area with occlusive dressings unless specifically instructed by a doctor.
Mechanism of Action
Clobetasol propionate exerts its therapeutic effects by inducing phospholipase A2 inhibitory proteins, collectively called lipocortins. It is thought that these proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. Arachidonic acid is released from membrane phospholipids by phospholipase A2.

Metabolites are excreted primarily by the kidneys, and to a lesser extent, in the bile.
Half life
Systemic half-life of absorbed clobetasol is not well established for topical use but is generally short, similar to other corticosteroids (several hours).
Absorption
The extent of percutaneous absorption of topical corticosteroids is determined by many factors including the vehicle, the integrity of the epidermal barrier, and the use of occlusive dressings. Systemic absorption can occur, especially with prolonged use, application to large surface areas, or under occlusive dressings. Inflamed and/or diseased skin may increase absorption.
Metabolism
Once absorbed, clobetasol propionate is metabolized primarily in the liver by various cytochrome P450 enzymes.

- Untreated bacterial, fungal, viral (e.g., herpes simplex, varicella), or parasitic skin infections

- Diaper rash in infants
Drug Interactions
Other Systemic Corticosteroids

Clobetasol is metabolized by CYP3A4. Concomitant administration of CYP3A4 inhibitors may increase systemic exposure to clobetasol, potentially leading to increased systemic side effects. Use with caution.

Overdose
