Description
Overview of the medicine
Clozema 0.05% Ointment contains Clobetasol Propionate, a highly potent corticosteroid, used for short-term treatment of inflammatory and pruritic manifestations of moderate to severe corticosteroid-responsive dermatoses.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Do not use for more than 2-4 consecutive weeks without medical review.
Elderly
Use with caution. Dosage adjustments may be necessary due to potential for increased systemic absorption and adverse effects.
Renal_impairment
No specific dosage adjustment required for topical application, but monitor for systemic effects with prolonged use on large areas.
How to Take
For external topical use only. Apply a thin film to the affected skin area and rub in gently. Wash hands after application unless hands are the treated area.
Mechanism of Action
Clobetasol propionate is a highly potent topical corticosteroid. It works by activating naturally occurring substances in the skin to reduce inflammation, itching, and redness. It binds to glucocorticoid receptors, inhibiting the release of arachidonic acid and subsequent formation of prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Anti-inflammatory and antipruritic effects begin within hours of application.
Excretion
Excreted mainly in the urine, with some excretion in the bile.
Half life
Plasma half-life of absorbed clobetasol propionate is approximately 3-5 hours, though systemic effects are often minimized with proper topical use.
Absorption
Systemic absorption occurs, especially with prolonged use, large areas, occlusive dressings, or damaged skin. Absorption varies depending on the site of application and skin integrity.
Metabolism
Metabolized primarily in the liver, similar to other corticosteroids.
Side Effects
Contraindications
- Hypersensitivity to clobetasol propionate or any component
- Untreated cutaneous infections (viral, bacterial, fungal)
- Rosacea
- Acne vulgaris
- Perioral dermatitis
- Pruritus without inflammation
- Children under 1 year of age
Drug Interactions
CYP3A4 Inhibitors
May increase systemic exposure to clobetasol, leading to increased risk of systemic side effects. Examples include ritonavir and itraconazole.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely. Chronic overdose or misuse (e.g., prolonged use on large areas, under occlusion) may lead to hypercortisolism (Cushing's syndrome) and adrenal suppression. Treatment is symptomatic and supportive; gradual withdrawal of the steroid may be necessary.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Clobetasol propionate should not be used in large amounts or for prolonged periods during lactation. Consult a doctor.
