Description
Overview of the medicine
Codomix is a fixed-dose combination medication containing Paracetamol, an analgesic and antipyretic, and Phenylephrine HCl, a nasal decongestant. It is used for the symptomatic relief of cold and flu, including headache, fever, body aches, and nasal congestion.
Uses & Indications
Dosage
Adults
1-2 tablets every 4-6 hours as needed, not exceeding 8 tablets in 24 hours. Do not exceed recommended dose.
Elderly
Use with caution. Dosage adjustment may be required in patients with renal or hepatic impairment.
Renal_impairment
Use with caution. Consult a physician. Dosage interval may need to be extended.
How to Take
Administer orally with water. Can be taken with or without food.
Mechanism of Action
Paracetamol is thought to act primarily in the CNS by inhibiting prostaglandin synthesis. Phenylephrine is an alpha-adrenergic agonist that causes vasoconstriction in the nasal mucosa, reducing swelling and congestion.
Pharmacokinetics
Onset
Paracetamol: 30-60 minutes; Phenylephrine: 15-30 minutes
Excretion
Paracetamol and its metabolites are excreted primarily in the urine. Phenylephrine and its metabolites are excreted in the urine.
Half life
Paracetamol: 1-4 hours; Phenylephrine: 2-3 hours
Absorption
Paracetamol is rapidly and well absorbed from the GI tract. Phenylephrine has variable oral absorption due to first-pass metabolism.
Metabolism
Paracetamol is extensively metabolized in the liver, primarily via glucuronidation and sulfation. Phenylephrine is metabolized in the liver and gut wall by monoamine oxidase (MAO).
Side Effects
Contraindications
- Hypersensitivity to paracetamol or phenylephrine
- Severe hypertension
- Severe coronary artery disease
- Patients taking MAO inhibitors or within 14 days of stopping them
Drug Interactions
Alcohol
Increased risk of liver damage with paracetamol.
Warfarin
Paracetamol may enhance the anticoagulant effect of warfarin with prolonged use or high doses.
Beta-blockers
May reduce the effect of beta-blockers and increase the risk of hypertension with phenylephrine.
MAO Inhibitors
Concomitant use with phenylephrine can lead to a hypertensive crisis.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose of paracetamol can cause severe liver damage. Overdose of phenylephrine can cause hypertension, tachycardia, and CNS stimulation. In case of overdose, seek immediate medical attention. Treatment includes supportive care, activated charcoal, and N-acetylcysteine (NAC) for paracetamol overdose.
Pregnancy & Lactation
Consult a doctor before use during pregnancy or lactation. Paracetamol is generally considered safe at recommended doses, but phenylephrine should be used with caution, especially in the first trimester and during breastfeeding due to vasoconstrictive effects.
