Description
Overview of the medicine
Cofnil Plus Syrup is indicated for the symptomatic relief of cough, nasal congestion, sneezing, and runny nose associated with the common cold and allergic rhinitis. It combines an antitussive (dextromethorphan), a decongestant (phenylephrine), and an antihistamine (triprolidine) to provide comprehensive relief.
Uses & Indications
Dosage
Adults
10 ml (2 teaspoonfuls) every 6 hours, not to exceed 4 doses in 24 hours.
Elderly
Use with caution. Dosage may need to be adjusted based on renal function and individual tolerance. Consult a doctor.
Renal_impairment

How to Take
Cofnil Plus Syrup is for oral administration only. It can be taken with or without food. Measure the dose accurately using a measuring spoon or cup.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to elevate the cough threshold. Phenylephrine is an alpha-adrenergic agonist that causes vasoconstriction in the nasal mucosa, reducing swelling and congestion. Triprolidine is an H1-receptor antagonist that blocks the action of histamine, thereby reducing sneezing, itching, and rhinorrhea.
Pharmacokinetics
Onset
Onset of action for decongestant and antihistamine effects is typically within 15-30 minutes. Antitussive effect starts within 15-60 minutes.
Excretion
Primarily excreted in urine as metabolites. Dextromethorphan is excreted as unchanged drug and metabolites. Phenylephrine and Triprolidine are also mainly excreted via urine.
Half life
Dextromethorphan: 1.2-3.9 hours. Phenylephrine: 2-3 hours. Triprolidine: Approximately 3-5 hours.
Absorption
Dextromethorphan and Triprolidine are well absorbed orally. Phenylephrine has variable oral absorption due to first-pass metabolism.
Metabolism
Dextromethorphan is metabolized in the liver by CYP2D6. Phenylephrine undergoes extensive first-pass metabolism in the gut wall and liver. Triprolidine is metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Dextromethorphan, Phenylephrine, Triprolidine, or any component of the formulation
- Patients currently taking or who have taken monoamine oxidase inhibitors (MAOIs) within the last 14 days
- Severe hypertension or coronary artery disease
- Narrow-angle glaucoma
- Urinary retention
- Concurrent use with other sympathomimetics or antihistamines
Drug Interactions
MAOIs
Can lead to hypertensive crisis, hyperpyrexia, and potentially fatal reactions. Avoid concurrent use and for 14 days after stopping MAOIs.
CNS Depressants
Increased sedation and CNS depression with alcohol, sedatives, tranquilizers, or other antihistamines.
Antihypertensives
Phenylephrine may reduce the efficacy of antihypertensive drugs.
CYP2D6 Inhibitors
Coadministration with inhibitors like fluoxetine, paroxetine, quinidine may increase dextromethorphan levels.
Tricyclic Antidepressants
May potentiate the effects of phenylephrine.
