Overview of the medicine
Cofno 10 mg Syrup is an antitussive medicine used to relieve dry cough and irritation of the throat. It works by suppressing the cough reflex in the brain.
Uses & Indications
Dosage
Adults
10-20 mg (5-10 ml) every 4-6 hours, not exceeding 120 mg (60 ml) in 24 hours.
Elderly
Similar to adult dosage, but caution for potential drug interactions and comorbidities.
Renal_impairment
No specific dose adjustment required, but use with caution.
How to Take
Oral administration. Use a measuring spoon or cup for accurate dosing. May be taken with or without food.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to elevate the cough threshold, thereby suppressing cough.

Excretion
Mainly excreted in the urine as unchanged drug and metabolites.

Side Effects
Contraindications
- •Hypersensitivity to Dextromethorphan or any component of the syrup
- •Concurrent use with MAOIs or within 14 days of discontinuing MAOIs
Drug Interactions
SSRIs/SNRIs
Increased risk of serotonin syndrome.
MAO Inhibitors
Concurrent use may lead to serotonin syndrome (nausea, vomiting, hyperthermia, muscle rigidity, mental status changes, hyperreflexia).
CYP2D6 Inhibitors (e.g., Amiodarone, Fluoxetine, Paroxetine, Quinidine)
These inhibit CYP2D6, potentially increasing dextromethorphan levels and adverse effects.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, dizziness, drowsiness, ataxia, nystagmus, lethargy, blurred vision, respiratory depression, and seizures. Treatment is supportive, potentially involving activated charcoal if recent ingestion, and naloxone for severe respiratory depression.

Pregnancy Category C. Use only if clearly needed and potential benefits outweigh risks. Consult doctor before use during lactation.
