Overview of the medicine
Colik 20 mg tablet contains Dicyclomine Hydrochloride, an antispasmodic medication used to treat symptoms of irritable bowel syndrome (IBS) such as cramps, pain, and bloating. It works by relaxing the smooth muscles of the stomach and intestine.
Uses & Indications
Dosage
Adults
Oral: Initially 20 mg three or four times daily; may increase to 40 mg three or four times daily if needed. Maximum 160 mg/day.
Elderly
Start with lower doses (e.g., 10 mg three times daily) and titrate carefully based on response and tolerability, due to increased susceptibility to anticholinergic effects.
Renal_impairment
Use with caution. Dose reduction may be necessary in severe renal impairment.
How to Take
Take the tablet orally with or without food. It can be taken before or after meals.
Mechanism of Action
Dicyclomine is an anticholinergic (antimuscarinic) agent that blocks the action of acetylcholine at muscarinic receptors on smooth muscle, thereby reducing muscle spasms and contractions in the gastrointestinal tract and diminishing secretory activity.

Contraindications
- Hypersensitivity to dicyclomine or any component
- Obstructive uropathy
- Obstructive disease of the gastrointestinal tract (e.g., achalasia, pyloroduodenal stenosis)
- Severe ulcerative colitis or toxic megacolon
- Reflux esophagitis
- Unstable cardiovascular status in acute hemorrhage
- Glaucoma
- Myasthenia gravis
- Infants less than 6 months of age
- Lactating mothers
Drug Interactions
Digoxin
May increase serum digoxin levels due to delayed gastric emptying. Monitor digoxin levels.
Antacids
May interfere with the absorption of dicyclomine; administer dicyclomine before meals and antacids after meals.
Metoclopramide, Cisapride
Antagonistic effects on GI motility.
Other anticholinergics (e.g., tricyclic antidepressants, phenothiazines)
May potentiate the anticholinergic effects (e.g., dry mouth, blurred vision, constipation).

Overdose
Symptoms of overdose include dry mouth, blurred vision, pupil dilation, tachycardia, urinary retention, headache, dizziness, nausea, vomiting, and CNS stimulation (e.g., restlessness, excitement, confusion, psychosis). Severe overdose may lead to coma, respiratory depression, and circulatory collapse. Management is supportive, and physostigmine may be considered in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Dicyclomine is excreted in breast milk and may cause adverse effects in nursing infants; therefore, it is contraindicated in lactating mothers.
