Description
Overview of the medicine
Contilex 250 mg Tablet is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg or 400 mg daily, given as a single dose or in two divided doses. For Contilex 250 mg, it is typically 250 mg twice daily, or as directed by the physician.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal function. For severe renal impairment, dosage adjustment is required.
Renal_impairment
For creatinine clearance <60 mL/min, dosage adjustment is necessary. Consult physician for specific recommendations.
How to Take
Contilex 250 mg tablet should be taken orally, with or without food. It can be taken with food to minimize potential gastrointestinal upset.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the final transpeptidation step of peptidoglycan synthesis.
Pharmacokinetics
Onset
1-2 hours
Excretion
Mainly via urine and bile. About 50% of the absorbed dose is excreted unchanged in the urine within 24 hours.
Half life
3-4 hours (average)
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Limited hepatic metabolism; primarily excreted as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe allergic reactions to penicillin (due to potential cross-reactivity).
Drug Interactions
Nifedipine
Concurrent use may increase cefixime peak levels and AUC.
Carbamazepine
Elevated carbamazepine levels have been reported when cefixime is co-administered. Monitor carbamazepine levels.
Oral Contraceptives
May rarely reduce the efficacy of oral contraceptives; advise patients to use additional barrier contraception if concerned.
Warfarin and other anticoagulants
May increase prothrombin time (PT) with or without bleeding. Close monitoring of INR is recommended.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Treatment should be symptomatic and supportive. Hemodialysis or peritoneal dialysis does not remove cefixime from the body in significant amounts.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts in human breast milk; use with caution in nursing mothers.
