Description
Overview of the medicine
Dapagliflozin is an SGLT2 inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus, and also indicated for heart failure and chronic kidney disease.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily, in the morning, with or without food. May be increased to 10 mg once daily based on efficacy and tolerability.
Elderly
No dosage adjustment required based on age alone. Monitor renal function.
Renal_impairment
eGFR 25 to <60 mL/min/1.73m2: 10 mg once daily for T2DM, HF, CKD. eGFR <25 mL/min/1.73m2: Not recommended for T2DM, but can be initiated or continued for HF/CKD under specialist supervision.
How to Take
Oral. Can be taken with or without food, preferably in the morning.
Mechanism of Action
Dapagliflozin inhibits SGLT2 in the renal tubules, reducing glucose reabsorption and increasing urinary glucose excretion, leading to lower blood glucose levels. It also has diuretic and natriuretic effects.

Onset
Within 30-60 minutes.
Excretion
Mainly renal (75%) and fecal (21%).
Half life
Approximately 12.9 hours.
Absorption
Rapidly absorbed, peak plasma concentration (Cmax) reached within 2 hours.
Metabolism
Primarily metabolized by UGT1A9 to dapagliflozin 3-O-glucuronide, an inactive metabolite.
Side Effects
Contraindications
- •Hypersensitivity to dapagliflozin or any excipients
- •Severe renal impairment (eGFR <25 mL/min/1.73m2 for T2DM) or end-stage renal disease, or patients on dialysis
Drug Interactions
Diuretics
May increase risk of dehydration and hypotension.
ACE inhibitors/ARBs
Increased risk of hypotension and hyperkalemia when used with other drugs affecting renal function.
Insulin and Insulin Secretagogues
Increased risk of hypoglycemia (dose reduction of insulin/secretagogue may be needed).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Management involves supportive measures. Gastric lavage or activated charcoal may be considered if recent ingestion. Hemodialysis is unlikely to be effective due to high protein binding.
Pregnancy & Lactation
Not recommended during pregnancy (Category C) as animal studies showed adverse effects. Avoid during lactation as it is unknown if dapagliflozin is excreted in human milk.
